    The facility is beautiful in the front, but a zoo in the back. My mom's nurse was the rudest person I ever could have met. I talked with the director and told her that I did not want that nurse near my mother again. The toilets were stopped up and the staff led my mother to another bathroom down the hall. And then on another instance, my sister let someone know that the toilets are still not flushing and the staff gave my sister a plunger. I would not recommend this facility to Lassie! I don't usually write in reviews, but we're talking about my mother, whom we are burying today. I had to call an ambulance because my mom was in so much pain and I literally had to wait almost an hour before a nurse answered the call light. Get it together, people lives are in your hands!

    Notice that there are no stars! DO NOT PLACE EVEN YOUR WORST ENEMY HERE!!!!

    There is no security what so ever. If someone has it out for a patient, they will be endangered.

    Grossly mismanaged.

    Nasty, nasty food.

    Sticky dirty floors; pests all over the place.

    Dead bodies are wheeled through the front door (with the body bags left open) -- during the day and work week!

    Do not send your lives ones here. My grandma was here 10 days and was supposed to be rehabilitating, but was left to die and so she did. She would call my mother numerous times at 3 in the morning stating that she was thirsty and wanted her to bring her water. They ignored my grandmas request. Left her in a soiled diaper she was trying to clean herself up and was covered in feces for hours until my aunts and sister came and cleaned her up.. She developed sepsis and pneumonia. When my family came to visit they noticed how awfully sick she was and no one had a clue!!!! The doctor on call said thanks for bringing this to our attention.. She died the next day peacefully in the hospital with excellent care form Hendricks county hospital staff. Please do not send your loved ones here. I am absolutely sick about the awful care she received. My grandma was the best woman I have known and I will miss her so much. I give this horrible place -2 million stars!!

    After using information obtained from websites like this one, we chose to place my dad in the Avon Health and Rehab facility in November 2011. At the time he was suffering from serious complications of a high grade brain tumor. When he was released from the hospital, he had considerable paralysis of the left side of his body and could not walk.

    The staff of the facility immediately scheduled a "care plan" meeting with me to discuss their plan of attach. Their conclusion was that they'd have him walking and using his left arm soon. To be honest, I was a little skeptical.

    My dad ended up spending about 7 weeks at the facility. By the time he left he was using his left arm and hand, and could walk sparingly without a walker.

    Just as important, my dad had regained the confidence and the will to fight on.

    In my opinion this had a lot to due with the dedication, and what I perceived as sincere concern, the staff had regarding my dad's recovery.

    I do not recommend for rehabilitation. If your loved one lands here, visit patient often, @ diffrent times of the day, & for multiple hours to form your own opinion. As time progresses, consider bringing in an independent source to evaluate your loved one's condition, as the patient may be capable of doing much more for themselves than this facility allows or encourages. They push meds!! View medical records, know every med prescribed, the dr. that prescribed it & why it's necessary. After 90 days Countryside recommended permanent long-term care 24/7 & oxygen for my loved one. After an outside RN evaluated the patient, she suggested the patient might recover better @ home so the family returned the patient home. Upon arriving home, the patient grabbed a walker & started walking. We were amazed as Countryside kept the patient confined to a bed or wheelchair with buzzers on both unless in physical therapy. Within 1 week of returning home, an in-home professional caregiver advised there was no need for oxygen anymore.

    The facility is clean and the staff courteous, but whenever I asked a question, the staff memeber I asked usually couldn't provide the answer and whoever could provide the answer wasn't available.

    Very slow response time to call lights and bed alarms which in one instance resulted in a serious fall. Poor quality food, slow service, food served cold, and no choice. Poor communication among staff and very difficult to call in and speak with a nurse. Messages left are rarely returned. Rough treatment by aides giving a shower on at least one occasion. Could not even bring the newspaper to the resident's room so the paper sat at the nurses' station all day several times. Generally an uncaring attitude with few exceptions and an unwillingness by management to make changes after being alerted to the problems. All they care about is your money. Stay away from this place if you care about your loved one!

    I believe this is a dangerous place to put your loved one. Chronically understaffed. Even after we met with four people at the facility and brought an ombudsman with us to try to address many issues we felt the facility fell short on (dropping and injuring my father was one of them, leaving him to sit in his own urine and feces for half hour at a time was another… there were more….) , the issues continue. Yesterday they woke my father out of a sound sleep at four a.m., dressed him, then put him back to bed in his day clothes. Then despite my father ringing the call button repeatedly to be helped to the bathroom, they ignored the calls, and ignored his yells and pleas for help. They never even got him out of bed so he could go to breakfast. They let him lay in his bed in urine-soaked clothing from 4am until after 10. That is SIX HOURS they ignored him. We filed two complaints with Indiana State Department of Health. We are taking him out of there and would not recommend this facility to anyone. I know I left a one-star rating. There was no way to rate it as a NEGATIVE FIVE STARS.
