  • Anderson Mill Health and Rehabilitation Center Austell, Georgia 30106

would not send anyone to this place unless you don't care. my wife was here and it was a problem from day on. the staff was nice and caring except the social worker. we were lied to on a lot of things as she is doing great ( they told the ins co she wasn't so they stopped payment) was suppose to get her up out of bed 2 time a day because of bed sores. they didn't. upon leaving they were to set up home health care but they didn't which has caused a ton of problems. don't sign any papers unless you understand what it says. they will lie about what it says or not tell you all it means. I do not trust the case worker or social director there.

Lacks proactive leadership. Leaders lack integrity. F ollow the money.

. No attention to detail in cleaning patient room and other areas where patients spend a lot of time.

Not enough staff to care for incontinent and disabled patients.

Not enough supplies to properly care for patients.

Many necessary supplies are not available and have not been for some time.

Unprofessional, rude staff who break many rules of professional nursing and jeopardize the well-being of the patients are allowed to bully those who want to do the right things for the patient and stay within the proper scope of practice.

This is a very sad place to live and work.

This place is not kept very clean, maybe it is the fact that it is outdated that makes it look dirty. My family member has been complaining that his medication is not given on time, he waited for several hours for them to distribute his meds. he said it took several hours for them is not changed him in a timely or more so sanitary manner he has stayed in a dirty diaper for 2 - 3 hours.

He stated that on the day he should have been taken to physical therapy, the aide told him that he would be back and never returned. My cousin is a veteran and has 3 purple hearts and should be treated better than he is.

Well first thing i would like to say is the nurses dont change peoples diapers like there soiled so if u bring your family here expect this for sure; second thing id mention is when your family needs therapy they dont really do that they get them out of bed to sit In front of TV my family still isnt better & was at this facility for 45 days third thing is your family needs more time & u have to work with front office people about assistance helping u getting him placed there is like a act of congress there names are marsha or Deborah williams they will argue with u & not get a thing accomplished forth thing i would say is clothes can never be found with or without there names. Over all I'd have to say this place is no good for no one not even a homeless animal. So please if u have other options go with the others unless u don't care.

I can't give a rate of -100. This place should not even be in operation. I can't see how it is safe to put 4 residents in 1 room. If there was a fire they would never be able to get the residence out with all the furniture and medical equipment in the way. The ceiling is caving in and the weight from the roof is bowing the windows. The A/C vents haven't been cleaned in about a year and they blow down on the residence food while they try to eat. When I was there to see my Father n Law a resident used the bathroom on herself and they let her sit there soaked for over 15 min. During this time there was 2 workers sitting behind the desk not worried about it. I said something and they tried to tell me she spilled a drink on herself. I was there when it happened and nothing was spilled. The 2nd day of his stay they didn't even feed my father n law till we said something around 9 PM. The pictures they show in this site is not what this place looks like. The walls are stained and damaged, there is molded vomit on the chair, the metal doors are rusted and the plug sockets are rusted as well. I'm pretty sure there is black mold where the ceiling is falling in one of the rooms. I don't believe the bathroom has been cleaned in a few months. There is urine standing around the base and is smells awful. I will be reporting this to the State, Fire Marshal and The Health Department. I have proof of every statement in this complaint. Please don't leave your loved ones in this place.

Wow, where do I begin. Was admitted at 6 pm on a Friday night, April 27, 2018 one hour before shift change. We did not see any staff for 2-1/2 hours. A female staff came in and told us some things . . . not much of any on boarding. We were told her meds were ordered per the hospital records and would arrive around 9 pm. That did not happen. Her meds arrived at noon the following day. She was in tremendous pain from three fractures and had come out of a hospital stay of 9 days. She was placed in a room on a wing of all men . . . some very verbal while sitting in wheelchairs in the hallway. I actually slept with a gun under my pillow that first night. Her shower scheduled was Tuesday and Thursday of which she got neither. On her discharge day we asked for a shower and did not receive. The food was deplorable to say the least. On Tuesday, mom got a roommate at 9 pm. There were multiple empty rooms so why did we get the roommate. My mom has medicare and Humana as supplement. Most everyone at this facility was on Medicaid . . . so why did WE get the roommate? No one could answer that question for me except we did not ask for a private room. No one even asked what type of room we wanted. On Wednesday she got pork (again), a small scoop of mashed potatoes, and three pieces of zucchini for dinner. She did not get her roll, her fruit, dessert or something to drink. I took the tray back to the cafeteria and they said if she's on the third floor (of which she was), then they always run out of food. Not once in the 7 days at this facility did we ever get the food that was posted in the hallway menu. Supposedly there was a doctor on staff. We never saw her nor did she make contact with me or my sister. If you go on their website, there is no way to leave a review. There is a phone number to call 866-300-3257 to give a verbal review. Try it . . . you'll get the same attitude that most of the staff had at this facility. Rude and uncaring. The only saving grace was the OT and PT department which is its own separate company. They were awesome and my mother was able to get out in 7 days. The director of mom's assisted living facility came for a care plan meeting (which was on our 5th day!) and decided that this place looked and acted like a homeless shelter and she didn't feel it was safe for my mother to stay. So we moved her three days before the facility wanted to release her. My question is "how is someone suppose to get well" in a facility that can't even provide a good meal? If you have to put your loved one at this place, make sure you are their advocate and you can visit during the day and in the evening.

I have been in health care for over 20 years and was surprised and disheartened at the care my family member received. The staff is lazy and many seem barely competent. They routinely ignore the patients and their families and spend much of the day sitting at the nurse’s station chatting.

Nurse call buttons were often ignored, sometimes for several hours, even though the patient room number was displayed at the station. Worse, “out of bed” alarms that ring when a patient gets out of bed or their wheel chair when they shouldn’t were also ignored. I myself helped a patient off the floor and back into her chair when her alarm had been ringing for almost 20 minutes.

My mother spent six weeks at Pruitt undergoing rehab and either I or one other family member visited every day. Not once, I repeat, not once, did any staff member acknowledge us, say hello, offer unsolicited updates on our mother’s condition or ask if there was anything they could help us with. They were at best indifferent but more often nasty and rude to both the patients and visitors.

The food is poor, was never hot and was dropped on the table or bed tray “as is”; the “warming” covers often not removed and the sealed items like juice and milk rarely opened, even for patients that could not do it for themselves. Although they ask that a menu request be filled out daily for the patients, on several occasions the wrong food was brought and, despite requests for adjustment, the correct meal never arrived.

Phone calls to the professional staff and managers were often not taken which I understand. People are busy. However, despite leaving messages, return calls were rarely done leaving family to recall multiple times.

Perhaps most distressing is the fact that on two occasions that I personally witnessed, the wrong medication was administered to my mother.

I could say more but you have the idea. Please consider an alternate facility for your loved ones if at all possible.

  • Northeast Atlanta Health and Rehabilitation Center Atlanta, Georgia 30319

Horrible! They left the urinal on the bedside table for days. They returned my dad to the wrong room and left him there… I had to find him. They filled out paperwork and checked the box that they had discussed it with me…. which they didnt.

  • Jonesboro Nursing and Rehabilitation Center Jonesboro, Georgia 30236

Think twice…I placed my parent for short-term rehab..within 3 days she was medicated courtesy of a LPN having access to the "emergency kit" . Good old Dr Harvey covered up for her. 4 days later I found my Mother in the same depends as I put on her at 10pm the next day at 11. Feces and urine were on her and all linens. She was again overmedicated I asked the nurse what did u give her? He said nothing.. I said then get an ambulance of course he did not he knew exactly what was wrong with her. She was admitted to the ER dehydrated and no surprise UTI. This 1 star rating is way to decent. DO NOT EVEN CONSIDER THIS PLACE

  • Signature Healthcare at Tower Road Marietta, Georgia 30060

I would never recommend this facility. Care from the physical Occupational therapy staff was good however that is the end of anything positive to say. Even that was not the 7 days a week as promised. They are severely under staffed and there is an attitude of I will get someone else to help you not my job. Simple things like a washcloth or fresh glass of water seem needlessly very difficult to atttain. A 40 minute wait for help to the bathroom was way to common. A weight gain of more than 12 pounds in a single week had to be called to the staffs attention. We were told at a care planning meeting that there was no release date planned and were led to believe there was expected to be another release planning meeting scheduled only to be told the following Fri. that she had to out by Mon. They released her with no homecare equipment at all.

  • Pruitthealth - Austell Austell, Georgia 30106

I GIVE THEM 1 STAR… MY AUNT WAS IN THIS FACILITY FOR 1 MONTH AND SHE WAS MAN HANDLED BY THE STAFF AT NIGHT.. WHEN SHE WAS DISCHARGED WE HAS TO TAKE HER TO THE ER DUE TO SHE HAD FLUID IN HER LUNGS IN WHICH THEY WERE NOT WATCHING AND SOME OTHER THINGS GOING ON. THE REHAB PEOPLE CALLED A WEEK AFTER SHE WAS DISCHAGED FOR COME OUT TO SEE HER , BY THAT TIME SHE WAS IN THE HOSPITAL. I WOULD ADVANCE NOT TO SENS ANY ONE TO THIS FACILITY.

  • Traditions Health and Rehabilitation Lithonia, Georgia 30058

This facility had the worse customer service of any center I visited. When I visited the place, the admissions coordinator was not available. I attempted to call the next day, but was only able to leave a message. My daughter went by there in the middle of the next day in an attempted to speak with, but the coordinator was unavailable then as well. After leaving several message for both the admissions coordinator and the director of admissions, I gave up. A week later, I still have not heard back from neither.

  • Abercorn Rehabilitation Center Savannah, Georgia 31419

I was sent here after surgery at St. Joseph's Hospital for rehab. Let me say firstly, that I have NO complaints about the rehab services provided. The rooms are teeny, tiny, and way too small for two patients. There were some staff members who went above and beyond expectations, and others who seemed to just be making time. I was provided with a bed side commode which was squeezed between the bed and the wall, and usually changed only after multiple requests and many hours had passed, and after the odor was intolerable. There were many problems about

getting my medicines when I was supposed to. I had no place to sit except for my wheelchair or a straight backed chair, which I could not sit in because of my back. If the bed had not been so bad, I believe I would have improved much more quickly. I was only "allowed" a shower twice a week. My bed was never changed the whole time I was there. I believe many of the problems were caused by under staffing. Once again, many of the staff members did excellent jobs, however, the system made that difficult.

There were long term patients yelling, threatening, wandering around in and out of my room. This is just the tip of the iceberg.

  • Elberta Health Care Warner Robins, Georgia 31093

Upper management does not seem to be able to resolve matters efficiently. Staff is nice but simple matters are never able to be handled adequately. Laundry is a big issue. privacy is a big issue. Very cramped patient conditions. State inspectors could do the families a big favor by popping in unannounced. Inspection time always runs smoothly since they know you are coming. Water is not in front of all patients at all times. Glorified babysitters and medication dispensers.

  • Harrington Park Health and Rehabilitation Augusta, Georgia 30907

This is a very beautiful and well equipped facility but they are NOT willing to work with people with memory issues. They kicked my mom out after only a few days. They should of said they don't want to work with patient's with memory issues in the beginning; because they knew about her mental state when they accepted her. They were extremely rude about her leaving. It is very hard to get someone placed in another facility; when they are not leaving a hospital. There's no way I would ever use this facility again or recommend it to anyone. If I taught my students the way they treat their patients then I would have been fired long ago. I think most of the problems were from the physical therapist not wanting to do their job and the rudeness of the social worker.

  • Pruitthealth - Covington Covington, Georgia 30015

NO meds given first 18 hours… except one wrong one. Did not turn patient who had bed sore. At all. Long time to get to patients. Bells ringing without being answered promptly. Had to ask for breakfast. Seriously consider another place. I hear the PT there is wonderful, though. And to their credit, there are a few nice people there. Other than that -- don't go.

  • Woodlands Health Care Midway, Georgia 31320

This company cares only for the zeroes after the dollar sign and keeping their fancy cars; no compassion for the residents whatsoever. It is a depressing place to work or live. The ceilings are moldy from poorly- or un-repaired leaks; the kitchen serves stale bread and fresh fruit maybe once a year; the scabies infestation was never acknowledged and thus persists; and the administration is incompetent and racist. Patient neglect is rampant: patients are frequently left in dirty diapers, dehydration is common, showers are basically non-existent, call bells are ignored for hours, and there have been way too many avoidable deaths. Unfortunately, they seem to pass state inspections, indicating that this company is full of incredibly skilled liars. This is a horrendous company and needs to be shut down. Do not send your family members or friends here. I would not wish it on my worst enemy.

  • Parkside Post Acute and Rehabilitation Snellville, Georgia 30078

When you go to Scepter for the first time, look at the faces of the residents. I did. You will see a calm peace on every face that comes from love and care given to them by the staff. When a resident speaks to a staff member, that person looks the resident in the eye and gives them their complete attention. The resident has a feeling of self-worth, knowing they are respected and what they are saying is important.

My Dad was at Scepter 4 years. He lived on the "Wings of Hope" with my Mother. The morning he died, many of the staff came in very early, hours before their shift, just to be there and with him and to show their respect for him. We were in awe of such kindness.

The executive staff knows exactly what is going on with each patient, down to the medications or changes thereof. They feel responsible for everything and make sure the caregivers have everything they need and are doing their jobs as expected. They have an open door policy for family members and residents. The nurses and CNAs are constantly on the move, NOT sitting, but checking on each person. A precise daily record is kept on each resident.

The activities and social events exceed all expectations. They are geared for each patient's ability. They know what each person enjoys, and stay with them to help.

My Mother has dementia. She has been in the " Wings of Hope" (a separate secured environment) at Scepter going on 5 years. They have a nurse and CNAs exclusively for them. When we visit, she is in the Community Room in the Wing of Hope. They have clothed tables to eat and for activities. It is always decorated for the next holiday. One whole side of the room is windows, making is a cheerful environment. She always tells me she is happy. On one of her better days, she asked me to make a list of each caregiver's name so she could thank them personally. She said: "I know I'm mostly in a "fog", but I also know these people are SO good to me."

Scepter is an answer to prayer. Scepter is not fancy or showy. The resident's rooms are quite large with large windows. It is extremely clean and smells good. The food is really exceptional, carefully prepared for each person's special needs. My Mother is a chocking hazard, so her food is chopped very small. By far it is the BEST place you can let your loved one feel they are HOME.
