I have been here for three days and yesterday I had a wonderful treatment that is free. A lady named Joe Joe came in and said I’m going to get you cleaned up . I’m gonna give you a bubble bath. I thought she was just kidding and going to take me for a shower. When we got there the shower mode been transformed into a beautiful spa. Walk in tub and all. Sure enough I got a bubble bath. Can’t tell you how many years it’s been since my back was washed. Played soothing music It was so relaxing and wonderful blew dried my hair and styled it what a wonderful experience and what a wonderful lady JoJo is. She goes all out to make a wonderful experience. Thank you JoJo it was great and made a wonderful friend

I was a patient for 2 months after knee replacement and would recommend you NEVER put a loved one in this facility. The CNAs for the most part do not care about the patients they are to busy texting on the phones. Most of the nurses try to do their best but some are just nasty to the patients. The nurses are LPNs and the head nurse lacks caring and if you try to complain she tells you your wrong and does not want to hear it. The food is horrible. The only positive is the rehab staff they actually care about the patients

The rehab is pretty outside but when you enter the place the people are rude starting with the blonde hair receptionist…The cna's and nurses are cold and rude…They have the residents sitting at the nurses station for hours….One old lady was sitting there smelling like 2 day old pre and poop…You walk past her and the smell will have you stomach turning inside out and upsided down…The take your personal belongings and give them to other people to use…They throw everything in boxes and tape them even if it is food that will spoil and the bugs was in the bag…They put poop in clothes in plastic bags and put them in a box and tape it…How do you treat the people's property like that?If you know like I know run pass that place and keep on going…I would not put my dog with those people to take care of….Don't go by the rating of the state it is not true you will see when you put your love one in the place that you made a mistake…Most of these places are just in it for the money and don't pay good so the service is terrible..How do you send a resident out for care and they lose them and don't know where they are….Then you call the residents family to ask where transportation drop them off at….Then had to call the transportation company to find the resident…All those people should be fired including the director of nursing sitting at a nice desk and no clue how the place is being ran….Things have to get better for the people that are in that smelly hell hole….The state should take the owners licences to run these places….Take the residents money and allow them to be treated like crap…God bless those poor people because they need help in getting out of that hell hole…They all have to answer to God one day…The elderly should be treated right because you never know how long your love one will be around…Last but not least my in laws make visits in the morning and they really got to see what that place was really like left on the toilet for along time….Left in poop and need to be changed…I received a call at work to be told about the place not being clean and you have to beg someone to take the resident off of the toilet…All about money not treating the people with respect….

Not a place for anyone to go to. My aunt is at Lake Mary Rehab. She is 91 yrs old and has dementia. She entered rehab after being in the hospital. I never get returned phone calls promised by various staff. Today I called to see if her lab results where back and was told yes her labs are fine. I was then told they were doing her discharge papers. When I spoke to the social worker at the facility a few days ago that the PT told me she was told my Aunt was being discharged and how I knew nothing about this and I had no longer term facility to send her to. The socialworker said you have plenty of time to find a place. Since conversation 2 days ago I have been looking for facilities that have an available bed for my aunt. Finding a long term facility takes time so I was surprised by the news they were discharging my aunt. I have called the corporate office after receiving this news and have not had returned call. I Also left 2 messages for the social worker at Lake Mary Rehab and have not heardback from her either.

My sister has been in this facility for three months and we are very pleased with the rehabilitation she has received while here. Most of the rehab staff are very caring and professional. However the food service is another story. My sister has several medical conditions that prohibit her from eating certain foods…oatmeal, pork, beef, eggs. upon her admission this was reported to and recorded by staff. While visiting her one day I noticed that her menu stated that she could not eat pork, but what was on her tray that day PORK. leaving her with nothing to eat. She and her daughter reported this to staff and it made no difference. Some of her recent meals consisted of 1. Mashed sweet potatoes and corn bread. 2. oven fried potatoes, rolls and a bowl of sherbet 3. egg salad sandwich and rolls 4. Mashed sweet potatoes, carrots and rolls. VERY NUTRITIOUS!?? Family members often take food to her just to be sure she has something decent to eat. The closer it gets to her discharge date, the worse her food choices get.

My grandparents were at this facility for one year. They both declined drastically both physically and cognitively during this time. My grandmother had several UTI infections due to staff not tending to her incontinent needs in a timely manner. There is no outside space for residents to enjoy or to get sunlight. When I asked if someone could take my grandfather out for 10 minutes (after he called me nearly in tears for feeling like he was in jail) the rude response I got was…."if we can find someone who has time ." I finally moved my grandparents into a Memory Care facility this week. I called and e-mailed several staff asking them to have medications and prescriptions ready to go, and to have my grandparent's personal belongings in order. Not only was nothing packed or organized, nothing was prepared for my grandparents to have their medications upon discharge. When I asked the nurse about the medications, her response was "Oh we don't do that here. The other place should have an emergency stash of medications if your grandparents need something." The staff at Sanford Healthcare and Rehab, particularly the Director and the Case Worker, were completely uncooperative with the new facility. The Case Worker lied and said "Their granddaughter said they didn't need their medications." Then the Case Worker refused to get on the phone with me when I called several times to address the lie. The new place had to personally contact the Nurse Practitioner personally after eight hours of getting no where with the Case Worker and Director, who also lied about the Nurse Practitioner calling the scripts in to the pharmacy, which never happened. The Nurse Practitioner stated she knew nothing about the situation, and she immediately called in the scripts so my grandparents could have their much-needed medications. BEWARE of this place, DO NOT leave your loved-ones in their care. They are liars and they do not take proper care of the residents.

I was there for rehab after having a hip replacement. I cannot find fault with anything about this place. The staff was caring and the food was excellent. I would not hesitate to reccoment this facility to anyone needing either nursing home care or short term rehab.

My father was admitted for what was intended to be a brief 1-2 week stay, for rehab, after a 4 day hospitalization for an infection. Although walking with a walker when he came in, he was only allowed to walk with rehab and not with staff because he was a fall risk. He was left sitting in an "activity room" for hours on end without any activities or even the tv on and not allowed to be in his room, again because he was a fall risk. One of the aides in this room refused to take him out so he and my mother could visit privately, so she had to do it, while being told by the aide that he was "BAD, BAD, BAD" My mother was told it would take the full 100 days for rehab because of his fall risk (not new-he has been falling for years). She insisted she take him home and got him out just short of three weeks. Not a moment to soon, as he had lost 11 pounds in 18 days and probably needed to go to the hospital again for dehydration if he hadn't left. He regained 6 pounds in 3 days, after getting home, and looked better than he had the entire time he was there. If he had actually needed to be in rehab. longer, we would have moved him to another facility.

RUUUUN!!!! DO NOT enter this so called Nursing and Rehab facility. There are a few good CNA's, but if they go on break forget get anything you need. Took days and days to get some of my Grandmothers meds while she scratched and itched herself until she bled. NEVER got her Ped socks for her swelling. After she did get better they wouldn't release her and held her as a financial hostage (meaning if we took her against medical advise her insurance wouldn't pay and she would have too). She also wasn't seen by a doctor in over a week, but they said she was to sick to be discharged, but apparently not sick enough to be seen by a Dr. Nurses don't give a crap about anything unless it's time to pass a pill. D.O.N is a joke. I'm SOOOOO Mad at the hospital that recommended this place because it's a DISGRACE, UNCLEAN and UNPROFESSIONAL!!!!!!!!!!!!!!!!!!!!!!!!

Never take anyone's word for a place you are looking to place a loved one into for care. Facilities are regulated by the State of Florida, and any nursing home or health care facility in that state can be found at: http://ahca.myflorida.com/ where the public can review open records to the public regarding complaint surveys, deficiency surveys, fines, etc. You owe it to your loved one to do your homework first. Then tour the facility. Pay attention to telltale signs of the pros and cons. Request a meeting with the administrator, director of nursing, and the social worker/social services. Research and tour other facilities to compare before making your final decision.

  • Life Care Center of Altamonte Springs Altamonte Springs, Florida 32701

I can not tell you in words how terrible I found this place on so many levels.

My relative was neglected and in my opinion abused. Half of the staff had no idea of what kind of care that should be given, their person was left dirty and the room was left dirty with days old food from meals left in the room.

Countless personal items gone never to be seen again with no recourse offered.

Rude staff that on many occasions flat out lied about what they did or did not do, know, see. Medical records lost and the wrong doctors seen.

When it was apparent to family there was something going on with the patent, the nurse on staff was asked about it, she stated "yea, he is going down hill fast" The family had to insist that they be sent to a doctor where it was confirmed that there was a very bad infection.

In my opinion You will be lied to and when you are not looking, at best your loved one will only be neglected.

I cannot begin to verbalize my complete contempt for this facility but I will give it a try! My relative was in for rehab but was placed on the longterm facility side which is the "ghetto" side of the building. Once you entered the residence halls, you are immediately greeted with the smell of urine and feces. I bought some deoderizer but was told that I could not keep it there according to MSDS guidelines. On several occasions, my relative sat in soiled diapers for over an hour and that was after we called the CNA for a diaper change so imagine the days when we were not there. My relative is a diabetic but the food that was on the plate was not what was on the menu. The "slum lord" that owns the facility doesn't have chairs for visitors, outdated decor (it looks like the "80's" threw up in the rooms). The assistants would barely communicate with my relative when they would change her and would just pull on her or move the bead without warning. I will say that there were a few great nurses and CNA's but the bad experiences FAR exceeded the good experiences. This place is severly understaffed, it is an attrocity to humanity to see how the elderly are treated at this facility. God help the patients trapped in this torcher chamber.

A friend of mine was sent to Lake Mary rehab to recover from a broken neck. While recovering they put him in a room with a man who had a fatal contagious bacterial infection! My freind requested to be move to another room but they denied him. He contracted the infection from that room mate. The doctor actually discharged my friend to go home knowing he had contracted that disease! The roommate died and now my friend though recovered from his neck injury is in critical condition due to the infection he contracted there. Lake Mary Rehab is horrible. Never let a loved one be admitted there.

  • Tuskawilla Nursing and Rehab Center Winter Springs, Florida 32708

The program of treatment for my mother was good. Therapy was pleasant but intense, which is a good thing. The negative part is communication and follow through. Visitors were told Mom had checked out when she hadn't. Doctors, therapists, nurses, and others seldom if ever called back. Promised materials never were forthcoming. I could never speak to anyone directly; instead, I was told to leave messages they would return, but that seldom happened. When I did get a call back, the individual would a person who was new to the program or to my mother's care and would therefore have no answers. Frustrating.

  • Consulate Health Care at West Altamonte Altamonte Springs, Florida 32714

This place is the worst. It serves "mystery meat" to patients, who do not receive the proper care from the lousy staff. Patients lay with dirty diapers for way too long, and essentially are not cared for unless they make a lot of noise. I'd say there is a high probability this place will be sued out of business.

We arrived at Lake Mary Rehab at night, after my husband had a Total Hip Replacement.

They did not have 2 of his BP meds, so his BP skyrocketed to 190 the next morning. Very different decisions about wound status. Attitude from personnel after we complained about the situation. We left the facility without being discharged, after notifying situation to insurance company and our orthopedic surgeon.

  • Island Lake Center Longwood, Florida 32750

This nursing home was a nightmare from the start. Not only is the entire staff outsourced from other countries (you can't understand any of them), they just don't care about their residents. Island Lake was the kind of nursing home you picture in a nightmare. All the residents are sitting the hallways in wheel chairs, with tounges hanging out of their mouths, half in the chair and half out, etc. They are so short staffed on top of having rude nurses, that it takes an hour for the nurse to come see you after you ask them to. I was there for a couple hours before I was meeting with the administration and social workers to get my relative released to a different facility. The floors were filthy, the toilet was falling apart in the bathroom, they refused to get patients dressed for breakfast, a friend of my relatives was there and he hadn't been given his meds for a few nights.

On top of all of the above, anytime you asked a nurse a question, and it could be as simple as "Do you have a comforter for the bed" they responded with, "I don't know, I will have to ask my supervisor" and then they never ask their supervisor. Family members are not allowed to spend the night, and unfortunantley my relative had to stay the night before the doctor would sign a release form. I called to check on him and the nurses would yell at me for calling, hang up on me, etc.

This is not a place to send a loved one. In fact, I wouldn't send my worst enemy to Island lakes. This is the most careless work I have ever seen in a medical facility, and the dirtiest. STAY AWAY!!! STAY FAR AWAY!!! There is a reason they have beds available in this place!!

1 star rating is generous. Sub standard care….isolation procedures never followed extending my mothers care over 6 weeks. Staff never worn gloves…..Mom was allowed in the dining/activity room for all meals exposing the other patients to her illness. My concerns were ignored by the social worker and charge nurses to the point I was basically called a lair that their isolation procedure were being followed. Still no definite answer but there could be an out of pocket expense to our family since she is over her amount of skilled nursing days and the assisted living facility will not take her back until this infection is gone. It takes days to get phone calls returned..and her care plan was never followed. We were never kept informed of her progress.

A year and a half ago I was a patient in rehab at this facility for 4mos. following a serious car accident..I was distressed to see so many negatives about this facility, sure it may have some rough spots as do a lot of places of its kind but all in all I was treated with dignity..the CNA's that assisted in my recovery were wonderful especially 4 in particular…I found Occupational therapy outstanding as it was really with this dept. that gave me the strength to recover same people all the time and they really care…..Physical therapy was also good but turnover was a problem which slowed up my femur recovery..The Doctor and his associate which do the rounds at this facility were the best I've ever seen! in fact I kept the primary care doctor that's how much I trusted him!

  • Lake Mary Health and Rehabilitation Center Lake Mary, Florida 32746

Took two weeks before admissions went over admission documents for signing. Never did have an orientation session. It was two weeks before a care plan meeting was set up. It appears Rehab does not work well with alzheimers patients who are aggressive and they called in a psychiatrist who quickly prescribes the drug Risperal instead of observing the patient for fear and confusion traits that first week. My mom had lived in her home for 80 years and was not used to a foreign place with people touching and directing her all the time. I know that dealing with aggression is very hard for everyone concerned but don't think a drug will fix everything. My mother was able to walk until her partial hip surgery. I sent her to this rehab center just to heal from surgery and be able to walk again. After three weeks she is barely able to walk and the excuse is her behavior. They are advising the family to either put her on this drug or they will contact Medicare and say the family is not cooperating with this facility. Her current meds were prescribed by a doctor who specializes in dementia and I trust him over this facility's cursory review of my mom's behavior.

  • Tuskawilla Nursing and Rehab Center Winter Springs, Florida 32708

Pretty on the surface and claims to be a 5 star facility. Behind the scenes my father was neglected. The therapist were busy shooting the breeze and had no personal interest or passion when it came time to do their job. It would take the nurses and techs up to 20 minutes or more to respond to my father pushing the call button. At least pop in to let him know you'll be with him n make sure there's no urgent need! I can't imaging what would have been if we were not visiting every day. I was constantly confronting, even the head nurse. Neither his pain or bed sores were being tended to nor monitored properly. We would often find him wet. I know it's not an easy job but he's been to other facilities were he was treated w much more compassion and smiles. I'm not one to complain but we also had a group meeting where I was assured if we kept him in an extra month, which meant out of pocket expenses, he would be taken care of and progress. It was a waste of $7,000 plus! We've kept him a week or so longer in other facilities but the thing that puzzles me is that Tuskawilla is the only place that charged us interest!?!? Try Adventist Care Center or Life Care Center of Altamonte. People have different experiences at different places. And this plays the biggest factor: The most important lesson I learned is to make frequent visits, "know" and keep tabs on your loved ones condition, medications, nurses and therapists. Acquaint yourself w everyone and let them know you are watching, holding them accountable and most importantly that you love your loved one. God bless you…..

  • Consulate Health Care at West Altamonte Altamonte Springs, Florida 32714

Terrible communication problem and administration is unorganized. Physical Therapy provided does little or nothing for patients. NEGLECT is the type of care patients receive at this facility. There is a handful of staff members that care and have to work to exhaustion because they pick up the slack for non-caring staff members, which is the majority.
