My mother was not cared for appropriately in this facility. She had a stage one bedsore when she arrived and left the facility with a stage 4 bedsore. The nursing care was substandard, especially on the weekends. She was left for days without dressing changes and since I live in another state was unable to check on her daily. The supposed medicaid expert never turned in any paperwork even though she told my sister-in-law repeatedly that she did. When day 20 of medicare arrived, they couldn't wait to push her out the door bedsore or not! This place looks nice, but the care is unacceptable as is the customer service.
This nursing home is not at all what it seems to be. The staff are over worked and there aren't enough of them to care for the patients. My father suffers for this I have to come there everyday to make sure he is getting attended too and I often find him in a mess. He recently fell and broke is hip here and if he had the care he needed then this could of been avoided.
When my mom first was admitted to Consulate Health Care at Lake Parker she was put in a bed without side rails. Then she was put out in the hallway and transferred from one bed to another Most of the nurses were very nice and helpful.
If it was lunch time or dinner and had to go the bathroom oh well lunch trays came before nursing care. They did not have a team to distribute trays. The nurses had to help with trays. I think this is poor management as nothing should come before patients.If I had known this was the practice of this place my mother would have never went there.
I looked the place over with the staff and they have just what I need for rehab. The staff are very nice. Looking forward to the 21 days I will be there. I am 6 ft 8 and they even fixed the bed to fit me. I like the place. Mark Dearwater
This is the second time my Mom has been in the Consulate this place is the best rehab home in Polk County ,they have helped my mom very well and when she was having trouble they sent her to the hospital fast. I spend a lot of time there with her, the staff is A+++++. I have no second thoughts about them I know they will treat her well my mom is very head strong and out spoken I know if they can put up with her and treat her like she's part of the family I can rest without worrying about her. My thanks to all the staff

I gave them 2 stars because they actually did a wonderful job with my Moms physical recovery. However, my Mother is more then that! Mentally and emotionally she was a wreck. My Mother was being heavily medicated to the point it was making her an emotional wreck which isn't hard since she has Dementia. I will say this…..consulate did a wonderful job in insuring her leg healed and totally ignored her emotional well being. Certain staff members were very good and some were not so good! You find this everywhere you go. This is my thoughts on it, Consulate of Winter Haven needs to treat their Patients as a whole person and not just the injured part. Healing my Mothers injury ,but letting her mentally break down is not healing ! Also when I called to get information a few members of the staff blew me off. Which red flagged me to be concerned.
My mother just discharged from this rehab center and had a wonderful experience. It appears as though there is new administration and new director of nursing since my mothers last stay almost a year ago. The difference has been night and day. Staff are friendly and eggar to assist, a chef greeted us within a day of our stay to arrange a meal plan catered to my moms likes and dislikes all within her doctors strict orders. Therapy staff was caring and management was quick to respond to any concerns. I chose this center for my mother because of Medicare's rating of 4 out of 5 stars. I would rate them a 5 out of 5 and recommend to anyone needing short term Rehab.
This is NOT a place to be when you are trying to get better after a accident with a brain injury. The LAST thing you want to hear & remember from the so called "Head of Staff" before you leave this place is " I HOPE YOU GET YOUR BRAIN BACK" and that is after SHE (yes SHE) said I was "Endangering the Community"
but she loved her Mercedes Benz 230 as much as herself & looks more than helping people. The "one star" is for the help from the volunteers,nurses and a couple staff members.
We chose this facility after careful consideration (including reading the reviews here) and was sorely disappointed in my mother's stay. Appearance was the only positive thing we found here. There was no warm fuzzy feeling in this place at all. The staff were not as friendly and personable as one would expect when it comes to patient care. Mother felt like she was dumped there and no one took the time to talk to her about her stay and what to expect as far as the facility (meal times, visitor policy, etc). This was her first time in such a place and she felt lost. There were a few specific issues that were immediately addressed when brought to the attention of Administration and for that I was grateful. But overall the experience was not a pleasant one.
Wonderful staff and facility. Very clean, professional, caring, and warming. I couldn't have hoped for my mother to be in a better place. I was very heartbroken when I was unable to give her the care and attention she needed. Thankfully, Astoria has been there for her and the staff does so much for her. She's very happy being here, it's wonderful to know she is in caring hands and happy.
Either they don't have supplies or they are expired. And that's include tube feeding nutrient. Poor food prep. If resident don't like food, and call the kitchen for something else, they always say they don't have it and that resident is forced to stay like that. Wound care either not documented or under classified so it doesn't show people have bed sites, and that is not from the nurses, management is to blame. They have an ongoing case of scabies in their memory unit that call rash so they don't have to report them to the health department, meanwhile the residents are itching like crazy and

Horrible facility!!! My grandmother is on the rehab unit and those nurses are so overworked with too many critical patients assigned to them that they can't get all of their patients taken care of properly. There is absolutely no support for the nurse or cna. One person can't do it all!!! I just pray my grandmother can get out of there alive.
Looking at this facility one would be skeptical of the quality of care they provide. However, after a few weeks of care for my father who is recovering from a heart attack and a spinal compression, there is no place I would rather him be, anywhere! The nursing staff is beyond professional and attentive, they are caring and effective. The quality of the physical therapy is phenomenal and has allowed my father to walk again! His emotional state was in a very dark place but the care he receives there has lifted his spirits immensely and he is happy!
I never understood assisted living commercials that stressed a patient's "dignity" until my father stayed here. Because "dignity" is exactly what he receives every day from every single wonderful staff member of The Crossroads. If I am ever in need of such a place, this is where i would want to stay. There are not enough good words to say about the exceptional quality of care that they provide.
By far the best place for Rehab in Winter Haven. Very knolagable staff and caring nurses.
My father died on November 25, 2012 from the staff neglecting him. They did not notify the family that my dad was not eating for 3-4 days. The staff continue to give him medicine. They focus him to take medicine and did not contact the family. I provided them several numbers and they still felled to contact the family. They even had the nerves to act concern when they called me that my father was dead. If anyone is planning to take their love one there think again. They do not have care about anyone and will not treat your family right. I found my father in feces at the nursing home and they took there time fixing the problem. That place need to be shut down for good.
I can't believe this facility doesn't have more stars then this. This is such a wonderful place for anyone. I would let my family reside here anytime. They have such a wonderful staff and are so friendly.
I wish there were more places like this one. Such sweet people! I love seein' the smiles on their faces when we walk in the door!!!!
I went in after surgery for therapy and was put in a long term care wing without an electric bed. Not good for someone that has to move around as much as possible.
I did not receive the therapy that I needed and they did not seem to have a plan for it. This was the second experience I had there within a year and the care was so different that you would've thought it was a completely different place. Did they change management? No consistency in the food. One day it was very good and the next inedible. At three in the morning when I rang for my pain medication and was told they could only give me half the dose because they ran out I went for 12
hours without any. Couldn't wait to go home!
My father is the most important person in my life. I didn't make the decision to have him put into an assisted living facility lightly. It wasn't an easy decision, because no place is good enough for him, but he has been living there for over a year now and his quality of life has drastically improved! The mere fact that he is now living with others who are going through the same struggles as him has had a huge impact. We are very pleased! Two thumbs up!
I can't believe the reviews I'm reading. I have had both of my parents there in the past and could not have been happier with their care. The staff was professional and friendly. They were very patient with my father who could be difficult and resistant to care , as a result he is now living independantly. The hospital had told us there was no hope. I would not hesitate to put my loved ones there again if the need arises.
This facility has seen a lot of changes in the past few months. The facility itself looks a lot nicer than it used to and walking around, there are new staff members that seem as though they actually care. I wanted to put in a review because I honestly believe this facility has gotten so much better. I have been happy with the level of customer serviced received here, but have had my reservations about referring someone because of the experiences of others. I feel so much more comfortable referring someone now. The new management seem to be working, as the facility seems in my eyes, to be so much better.
My mom was admitted at Astoria for rehab after a surgery. The grounds are beautiful and most of the staff is very nice and polite. With any rehab/nursing facility, I had to sometimes remind the staff of what mom needed to make her comfortable. I also made sure to follow up with each nurse as shifts changed and the unit charge nurse as well just in case something I communicated slipped through the crakcs. I would recommend doing this at any facility. Overall I was pleased with the patient care, service and amenities.
