This place is a dump and is way to small for todays active resident. They accept anyone just to fill the beds. The only plus is the view. It needs to be torn down .

Please don't take your loved ones here. There is literally no nursing care. They are so understaffed. Call buttons are ignored. The night staff is abominable. Physicians are never called when needed. My brother had wounds on his feet. They sent their 'wound nurse' once in a while…who put cream on the wounds and wrapped them. I finally moved him to another facility where a wound DOCTOR saw him. Conclusion: his leg was amputated due to the poor care he received at this rehab center. The rehab staff was great! They tried. If all you need is rehab this may be OK, but watch your loved one carefully if any medical attention is needed. There is none. Beware. We thought we were going to a 'Freedom Square' facility. It is NOT. It is Brookdale…not Freedom Square which has a good reputation for its care.

They meet the minimum requirements for staffing. Response time is poor even with a family member in the room. They accept patients from the community and their gym is too small to accommodate both of their inpatients and outpatients. They were rehabbing patients outside in the heat on their patio. I witnessed a patient in a wheelchair who had a pulley attached to a door (to a storage closet for walkers, etc.) on 9/28/16 She was working really hard and I praised her. Then a staff member asked her to stop her rehab as the staff member had to open the door to the closet. They took the pulley down and whisked the patient away. The communication is poor between family, patient and staff members. The patients may or may not get their allotted 2 showers a week depending upon whether they have a family member to advocate. My Mother waited 30 minutes in the middle of the night for an aide to assist her to the bathroom. She asked when she was admitted for only female aides and the aide that finally came was a male. They are attempting to clean up their act since I advised I contacted an Ombudsman and will be contacting AHCA and the Department of Insurance but this will be short-lived. Do not sent your parent(s) there.

Carrington Place in St. Petersburg is for sure a place to stay away from. The food is horrible, they are lazy and will not take them to the dining room to eat or if eating in the room it arrives hours late and is cold. The entire wardrobe of clothes did not return to the room and lost forever, they do not care. The care is pathetic, no changing of the diapers or bedding, no time with the patient, no doctor to see the patient, no showers, and not one staff member cares. Every aid, tech, CNA, nurse, or any staff member says they do not know and passes the buck to another and another and another. This place is horrible - do not sent your loved one here. Cars were broken into so leave no valuables in the car, no reasonable parking whatsoever. The hand rails to either go up the steep steps or the handicap ramp are sticky and gross. The ashtrays in the front or the smoking area are not cleaned. Disgusting place!

Extremely SHORT STAFFED

PT staff overworked and stressed.

CNAs were the only good thing here.

Social Workers and Administrators ignored complaints as did the corporate office!

Horrible experience! NO shower in 12 days! The aides are not cheerful or helpful. They come to help whenever they get around to it - press your buzzer and wait a couple of hours. Took ALL my clothes to the laundry and never received any of them back - 11 outfits - GONE!

Someone I knew was staying here because she had bacteria in the heart and was on antibiotics every 4 hours one of the male nurses was consistently sending messages to the patient so I went through the phone and read messages that they were having sex and doing other things. I believe this is very wrong and hope the director fixes the problem.

This is one of the filthiest places I have ever been in my life. There is caked on dirt, food, feces etc.. in the hallways and rooms, the PT room has blood or another dried substance on the wall near the sanitizing station. My husband (hip replacement therapy) was supposed to be admitted Sunday 2/15/15 first thing in the am. The transport driver came to BAYFRONT hospital to pick him up at 8:30am and was told to turn around and go back as the room at Lexington Rehab Rapid Recovery had to be sanitized. When he finally was admitted at 4:30pm to the "sanitized" room, the wallpaper was peeling from the walls, bugs were climbing on the walls and the baseboards and corners of the room were black from months and months of filth. I HAVE PICTURES OF ALL OF THIS. The food portions are miniscule and the food is all processed loaded with salt, no fresh veggies or fruit is served. Bfast today was corned beef hash with toast and oj. Who eats corned beef hash these days. This facility must be experiencing financial difficulties. All but a few of the nurses were helpful, everyone walks around looking like they are busy but most of them are sleeping in the TV room which is also filthy and cups from starbucks are sitting there from days. We asked to see the Head Nurse to get an early discharged since he had been there since Sunday and had only 2 therapy sessions. The staff kept telling us they called her and 24 hours went by and we never heard from Barbara and finally I had to raise my voice to get some answers and she came to see us and told us the staff never contacted her. We got the runaround the whole time he was there. We had to request he have his bandages changed. I was under the impression that when a patient is leaving a hospital to be discharged to a rehab facility that a list of facilities need to be presented to the patient. Christine who Bayfront hires to assist patients in this procedure never presented a list, she mentioned Lexington, Apollo and one other, not up to par facility. If my husband did not have me an advocate (squeaky wheel) to get some action to this shoddy facility I am sure he would have still been in there. Don't allow your loved ones to go to this place.

i was brought to countryside rehab in march,from being transfered over by via van from morton plant hospital after accident, in my stay at rehab, i experienced not pleasant conditions and care… the second day at my stay ,physical therapist was to busy on phone to watch me..in short note result falling over…hours later did hit the ground and having dr care or any immediate pain relief was non existant at best done was few xrays,..and results of fall started a case of arthritis in a severely broken leg , and to a separate issue my lower dentures was mistakingly confused as garbage with food tray and thrown out,…and the rehab center has arranged for dentist work to replace dentures,..and im process which has taken better than 3 months to replace ,..having a lateral affect causing further damage to my digestive system,..and a choke hazard but yet no teeth ..so rating my stay as less than 1.

I wouldn't give this place a one star RUN RUN RUN do not bring a loved one here they almost killed my uncle they have no positive emotion for the residents. They are very unprofessional and the care is horrible I promise you I am being completely honest this place is horrible if you love your family member do not let them stay at this nursing home. Since my uncle left on deaths door he is in another facility doing so much better after being in the hospital because of the neglect after two months. Horrible place I cant say it enough

I do not recommend this facility to anyone that is considering placing their loved one in a nursing home. Countryside Rehab and Nursing facility does not treat their residents with the appropriate care they need to receive. No only have I reported many incident regarding my loved one to the staff I had to make several complaint to the State as well. They cover up resident accidents by not informing the family, I have seen CNA's neglect their patience they are assigned to and watch poor care given on numerous occasions. Nurse come and go like a revolving door. This place needs new management

In addition, their therapy program is more of a cookie cutter, one goal for everyone which basically consisted of thowing balls back and forth.

This facility is terrible when it comes to patients needing trache care. My mother was breathing off the ventilator for nearly a month. Sent her here for continued rehab and she passed away 5 days later! They don't like to suction trache patients. And because of that my mother aspirated. The late night nurses are lazy and don't check on the patients nor answer the phones when you call late night to check on your loved ones. I hate I sent my mother to this place. No compassion when she passed either. Just a simple call stating she had passed, yet when I arrived shortly after she was already cold!
