  • North Florida Rehabilitation and Specialty Care Gainesville, Florida 32605

My brother went into this facility and could walk by himself with a walker. When he was discharged (28 days later) he cannot walk even with a walker. He was wheel chair bound. He is a very controlled diabetic. He was given a high carb. diet and the third day there as a pt. I walked into his room and the nurse was there to give my brother insulin. I wish I could give this place 0 star. THE CARE AT THIS FACILITY WAS HELLISH.

  • Parklands Care Center Gainesville, Florida 32601

I've never been inside, but I live next door to this place, and the staff are frankly terrible neighbors. If multiple individuals working there are willing to almost run down and then curse out pedestrians in the area, I can't imagine them being worthwhile at all for patient care. The nursing staff there have universally been jerks when I've been forced to interact with them outside the facility, and that's got to reflect on their quality of care.

  • Plaza Health and Rehab Gainesville, Florida 32608

My husband was transported here from the hospital for rehab on a Friday evening. The room he was to share was dirty, dingy & smelled of urine. The bathroom was dirty & had visible feces on the wall & hand-rail. We were told they probably wouldn't have all his medications until Monday ("We're not a pharmacy, they said) unless we can borrow some. Rehab wouldn't start until Monday also, as "They're the big shots & they don't work on weekends." Walking the halls & peering into rooms, it looked like people had just been left there to wither away & die.

WE REFUSED TO STAY & LEFT IMMEDIATELY. Even medical staff said to leave.

It is horrible that conditions like this exist and that facilities as these are allowed to remain open!

This facility is very clean, the staff are friendly and helpful. My mother was here for a fractured hip and we never thought she would walk again-The staff were able to get her back on her feet and home within a short period of time. I would recommend this facility to anyone that needs rehab!

DO NOT EVER LET ANYONE YOU LOVE COME HERE!!!! My mother went into this facility at a weight of 96 lbs and she could walk by herself with a walker. When she was discharged she weighed 80 lbs and cannot walk even with a walker. She had a UTI, a compression fracture of her spine from a fall in her room at the rehab, bedsores, and a 3-inch-diameter cyst on her back. No one told me about these conditions, I found out when she had to go to the ER one day after her discharge.

The doctor assigned to my mom, Dr. Erbay, refused to return my calls. I called his office to leave a message for him to call me and was told that if I wanted to speak to him I would have to sit at the rehab and wait for him to come. She could not tell me when he came to the rehab. I work full-time and could not sit at the rehab during the day, waiting for the doctor to come. My mom got sick while she was in the rehab, and no one called to tell me. She was so weak that she could not get out of bed, but they would leave her food on a table across the room and then tell me that she refused to eat. One day my daughter was visiting her and the aide took my mom's food tray away when she was still trying to eat!

Three individuals came into my mother-in-laws room with the sole purpose of severing an 18 year old family bond. My mother-in-law and I do not see eye to eye on everything but, we love each other. One of her sons visited her when she was in intensive care and the other has never visited. I on the other hand, have been involved in her life for nineteen years (since 1996). All three of the individuals (since I cannot divulge who I am talking about) should be fired for coming into my mother-in-laws room and taking part in what can only be called a "ruckus." The first person said to me: "You aren't supposed to be here" several times. I was aghast at the fact that me and my mother-in-laws pristine day was under attack by a pit bull of a woman. I asked her to leave the room as my mother-in-law has been treated for anxiety for years. I told her I would speak to the administrator but my conversation with her was concluded. She told me: "This is my facility" and refused to leave my mother-in-laws room until I told her "I am going to sue you if you do not leave." She came back with her other two people and the three of them tried to do the same thing. Start an argument in front of my mother-in-law. Being an African-American man, I knew they thought they were going to have grounds for calling the police except I stayed calm, cool and collected and let them do all the noise-making. Suddenly one individual says, "you are right, let's go to my office and have a meeting." I said: "I would be happy except I am attending no meeting that will be attended by individuals 1 & 2 as they had already attempted to use "unsavory" tactics to sever my bond with my mother-in-law. Because I have dignity and some education, these two individuals have taken it upon themselves to engage in character assassination instead of providing proper care for my mother-in-law.

They are incensed at the fact that I found feces and urine on the wall in my mother-in-laws room. My fiancé found that a CNA left a feces on the outside of my mother-in-laws ostomy bag and on her bedding. We kept records of her nonexistent turn schedule as well as the fact that instead of being put on a bed pan, she would always have her diaper changed without the proper attention given to cleaning my mother-in-law. We are incensed at the lack of sensitivity many staff members have shown during our family's most trying time.

  • Park Meadows Health and Rehabilitation Center Gainesville, Florida 32608

This place is so mismanaged and treat people like cattle. My mom was left in bed all day and not dressed, fed or had rehab for one day. I was told that was because the head nurse on that floor returned form maternity leave. What a lame excuse. Stay clear of this dump

  • North Florida Rehabilitation and Specialty Care Gainesville, Florida 32605

North Florida Rehabilitation and Specialty Care was recommended to us for my moms rehab care after a broken hip. This facility is a disgrace for care for the elderly. My mom was there a total of 2 weeks, 4 weeks was recommended, but we wanted her discharged as soon as possible. Her safety and health was at risk being in this facility.

After summoning for bathroom assistance and waiting almost an hour, she finally got up herself, and then fell when returning to bed. She laid on the floor vocally calling for help for over 35 mins. Her roommate summoned for help, but no one came. Her roommate then called her son, at home, and asked him to call the nurses station and tell them that immediate help was needed in their room. They finally came. This incident was never reported.

I filed a grievance report but no feed back was ever reported back to us.

My mother is a diabetic and has a strict diet. Over and over my mom was given meals with sugar lades desserts, and lots of potatoes, rice, pastas, which spiked her sugar levels. Spoke with dietician, but nothing was ever done.

Her medications, which she needed to take a half hour after breakfast, came at 11:00. Again, she is a diabetic and needs these medications.

She was given pain medication, that she never requested. It had been requested by her roommate, but given to her by mistake.

Medications that she was suppose to receive in the evening, she never received, although it was recorded that they were given.

My mom had diarrhea and didn't quite make it to the toilet, as a result there was a mess all over the toilet, and floor. I cleaned it up as best I could and was told that someone would be in to clean up the rest. Four days later, after I was fed up with the smell and mess, I talked to the head of cleaning and maintenance who came finally came in and cleaned it.

Her linens which were dirty, were never changed until after more than a week, I requested it done.

In her 16 day stay there, she was given 2 showers. The second one coming after 12 days after her first one. My mom would ask them over, over to please take her for a shower, but they would ignore her and give her excuses as to why they couldn't. I was told by one of the staff members, that it was recoded that she had been given 5 showers!

We were there with her from 6:00 a.m. til 8:00 p.m. daily with a few breaks in between. Don't know how much more my mom would of suffered if we hadn't been there.

On a good note, the Physical Therapy department staff was excellent! They are the only reason she remained there to receive their help til we felt confident to have her back at home.

If I can give 0 stars, I would. I base my rating on the first (and hopefully, the last) time I visited GHCC. First of all, the sign on the street that says Gainesville Health Care Center should give enough warning NOT to go to this nursing home. It looks like it's an abandoned sign from the 80s. Weathered, discolored, and barely noticeable from the street. Second, staff was very informal and casual. Immediately stepping out the elevator, I noticed a young woman taking a resident's blood pressure. She was wearing jeans, cowboy boots, and a t-shirt. I would not have known she was a CNA, if I had not seen her tiny badge that said she was one because of how she was dressed. Third, she and other staff held conversations with each other from a distance, thereby talking loudly up and down the hallways. No shame in yelling across residents. I understand it is not a hospital, but there should not be a reason why staff should look or act questionable whether they work there or not. I also witnessed VERY POOR transfer techniques used on residents. For example, an older gentleman resident with a movement disorder needed to go to bed and so his aide came by to help him. I saw the greatest fear in his eyes when the CNA hauled him up by his armpits and swung him on the bed. Fourth, staff was unprofessional with residents' families. I witnessed a CNA VERY RUDELY addressing an older woman who was tending to her husband. This particular CNA threatened she was going to "quit in 2 minutes" and that she "ain't pickin another person up off the floor." From this point on, it was CLEAR that a lot (IF NOT, most) of staff who work at GHCC do not offer the quality "care" at this center, but just do it to make money. VERY VERY SAD but this is what I witnessed. I left GHCC with such a heavy heart for all the residents and their families.

I understand the consequences of leaving residents no other place to go, but I hope this place gets shut down (or makes IMMEDIATE DRASTIC changes). Residents and their families deserve more respect than this. I feel for them.

I had opted to have my husband place at this facility after a long hospital stay. At first glance I felt that this was a positive decision my on my part and my husband would get the care he needed in order to come home.

The daytime personnel were very friendly and caring however the after hours personnel seemed almost hostile when I called and questioned them about their care or requested to check on my husband. As days turned into nights, I realized that my husband was not given his medicine and was having trouble breathing as a result of this. My husband called me to request I bring his medicine from home and I did so. Talking to the nighttime nurse I pointed out to her that this was supposed to be given on a daily basis and she could not have been more hostile telling me that I had no right to administer medicine to him that they had not given for a full 8 days. Making matters worse they let him fall causing trauma to his ribs (according to the hospital he had to re-admitted to) and upon being admitted to hospital, they actually took pictures of the bedsore the facility failed to treat or prevent

I can only suggest to Woodlands that they look closely at their overnight personnel and reinforce to them that they, the overnight personnel actually have a job to do. It is unforgivable that they overlooked the basics and neglected to provide the care and dignity each patient deserves when they put their well being under their care.

This place is absolutely appalling. PLEASE do not send any of your friends or family here. The poor care at GHCC is what I believe contributed to an early death of a close friend of mine who was paralyzed. She was constantly threatened by staff members, witnessed other residents who were less mentally with it laying in their own feces for hours at a time, and was neglected again and again when she needed help. In her memory I write this review, praying that no one else will pass away early or be severely neglected as a result of the lack of care at GHCC.

  • Signature Healthcare of Gainesville Gainesville, Florida 32607

My mother recently stayed at Signature in Gainesville 9/12 and we were pleasantly surprised by the warmth and compassion towards my mother and family. she felt comfortable and a we became attached with her care giver's. I was educated by the nursing and rehab team an greeted with a smile during my visits. thank you Signature!

  • Parklands Care Center Gainesville, Florida 32601

I've been to several local nursing centers as a volunteer and I'd say that parklands ranks somewhere in the middle.

While the staff is overworked and sometimes residents have to wait longer than they should for services, The place is fairly clean and many on staff do their best to take care of the residents with love and compassion.

  • Palm Garden of Gainesville Gainesville, Florida 32607

The facility is clean. Attention & response to residents when In need of assistance needs improvement. Meals are acceptable & are served in a timely manner. Response to call lights is very poor. The signs on the wall of every room say "Call Don't Fall" are a joke with the slow response time. Ordering residents to do tasks they are not ready to do on their own contributes to the levity of the call don't fall signs. Some nurses seem not to be able to handle required .medical procedures. Over all the facility is pretty good, but patient service does need attention.

  • Signature Healthcare of Gainesville Gainesville, Florida 32607

As a whole staff unprofessional talking poorly of other facilitys in the area was not what a family want to hear when getting a tour. Staff deem to be all about their own careers family does not care if your in school or recently obtained your RN. This building is set off the side of the road like a hole in the wall and do not partake in activities with residents outside the buiding, no outings? Family watched as staff ate off trays of patients?

  • Parklands Care Center Gainesville, Florida 32601

Before my client (who was former inspector for nursing homes) came to parkland he was told they 'torture' their residents. He didn't believe them. When he went there as a patient they knew his former occupation and they ignored the call bell. They ease dropped in his room using a speaker and he could hear them talking about it in the hallway. They also screamed out his name at 2am and parked car outside his window purposefully playing loud music. I would avoid this nursing home like the plague.

I have a relative in this rehab and I cant wait for the day I cant get her out and into a new one! THis place is horrible. There is maybe 1 out of 10 CNA's that I have heard or seen that is actually qualified and friendly enough to take care of the people we love that unfortunately have to live here. If you cant follow through with what you went to school for, GET A NEW JOB!
