I give this facility a 1. Only for the nursing staff. The rest 0. The food is horrible. They forget about the people and run out of food. My mother got toast due to shortage of food. I have been there personally and could not cut thru pork chops… They need new management in the nutrition service dept.

Please check out the current medicare rating on this facility. Don't rely on past reputation. My parent was in the rehabilitation wing and within 4 days had aspiration pnuemonia, a huge bed sore, kidney failure from dehydration, and almost died.

5 years before my other parent was there and did fairly well, but things have changed. Always check current medical ratings, check on the place without warning, and compare number of nurses from place to place.

Hospitals don't keep seniors long these days, so they are often sicker and more vulnerable than they used to be for rehabilitation. Be careful with your elderly loved ones.

This place is literally HELL on earth. Horrible staff. The only staff member that actually cared for the patients was Jan Eyre (CEO) but she is now gone. Now, there is nothing that can be done when the CNAs are verbally and physically abusive. 2 CNAs named Natalie and Rocio are physically, mentally, and emotionally abusive towards patients. When I brought something up with John Adams the (COO) all he did was say there was going to be an investigation. Still haven't heard anything back and those 2 CNAs are still there. Very horrible place to have a loved one at. PLEASE DO NOT TAKE YOUR LOVED ONE THERE. It will save you lots and lots of stress but especially tears.

I am not impressed with this facility at all. My Father is in the facility. Because of his medical insurance I can not get him in a nicer Facility. There has been numerous occassions of neglect and lack of professional care from the Doctors. The nursing Staff run the place and they do what they can to help. Unless You are the Sqweaky wheel you will not get the care needed to accomplish anything other then turning a loved one over for long term care and drop off. I have been told from the doctor in the facility that medicaid will not let them help get them better….. Only maintain that they do not get worse…. Dictation of Medicaid only allows the doctor to see them once a month……. WELL It is my understanding that if there medical needs require more then the doctors is allowed to take care of them…. As well as that goes We have seen specialists to get his care taken care of yet the doctor declined doing what was asked to be done for his care while at the facility. 2 weeks later he was not getting the treatment perscribed by the specialist. I had to get the specialist and the nursing staff to talk and force the MD to even talk to the specialist to get him on the right track. The MD seemed to think that the very dangerous condition my father was in didnt warrent discussing and figuring out the propper treatment but instead decided to just ignore everything. Had I not asked I would have never known that they did not follow through and the medications had not been changed or treatment not followed. This neglect is in my opinion not right in any way shape or form. IT makes me feel the doctor in charge of my fathers care is just collecting a paycheck and really doesnt have patients condition in heart.

Please do not put your loved ones here. They do not notify you of major medication changes, changes they made in their diet, falls and injuries, we experienced many falls and they do not answer bed alarms.Patients who need to be assisted in feeding is given finger foods instead of regular meals. I have seen the more active alzhiemer patients drugged if giving the staff a hard time when no family is around. They lose the glasses, dentures and clothes of the loved ones and they are never replaced. They pretend to care and tell you what you want to hear but never follow through with corrective actions. They seem to get alot of grief from the state but nothing ever happens to them. There are mice running around and the stench is horrible. We are miserable trying to move our loved one as memory units are sparse. You will have many tears and frustration getting humane care.

We had a good experience with my father at this facility. He was there for rehab for a month after a hospital stay/surgery. The CNAs were patient and kind and gave good care. The fact that they spoke English well was a benefit….no difficulty understanding them. The food offered choices and variety. Patients were served in their rooms or went to the dining area. There seemed to be a variety of activities offered that would appeal to permanent residents. The physical therapy was available 6 days a week and it benefited him.

I would not recommend this facility to anyone. Our family needed help and guidance from these people regarding care for our father and all we got was double-talk and the run-around. Finding quality care is difficult enough on families with elderly loved ones. Rather than treating our family as adversaries, they should have tried to work with us to provide the best care possible under difficult circumstances.

I am a former employee of this facility I worked there for over four years. This is one of the worst facility's that you could put your loved ones in. Management is incompetent only out for making money. They let tons of good nurses and cna's that actually care about the residents go to get in bran new aids that will do what they want. They DO NOT put the residents first and in a nursing facility like this that is why I thought we were there. There is a few good people but few and far between.

The staff at Malley are very dedicated and experienced. Many have been there for a number of years. We have a relative at Malley (and was also at the Kindred facility downtown), and they have always provided excellent care and great communications. I would not hesitate to recommend the Malley facility to anyone.

My mother was in Elms Haven for rehab for 25 days. It was by far lacking in every department. For one, they told us initially that Mom could only stay for 20 days, check out for 3 then come back in, we didn't find out until day 18 that there was a work around and that they actually had a Social Worker there. In the 25 days, she was only allowed 3 showers even though she had bed sores. We had popsicles in the employees’ freezer for her, 3 fresh boxes were dropped off, with her name on them, she didn't eat any of them for a day or two, my brother went to get her one and a box and a half was gone. My brother would deal with the Admin and answer their billing questions, then someone different would call him and ask the same questions 3 days later. The medical staff was incompetent and totally unprofessional. They would give Mom test results that she had already received a week prior. They did nothing to help her condition and when we brought up the subject of moving her to hospice, they scrambled every way to next Sunday to try and keep her in there. They didn’t care about her; they only cared about keeping that bed filled, totally pathetic!!!

Staff is constantly losing my grandfathers clothes, and when we did get them back they reeked of urine and feces even after we trying washing them several times. They let their laundry build up and don't return clothing for days or possibly never in our case. Some of his personal items have dissapeared as well.

My grandmother is very ill and so her power of attorney has asked the nursing staff to contact him in case there is an emergency instead of my grandmother. In spite of this, they contacted my grandmother late at night to relay some disturbing news and she ended up crying all night.

Also when we try to contact Elms Haven regarding a bill or insurance information, they do not respond to our messages.

At this point, we wish that we had kept my grandfather at home and taken care of him ourselves. Especially after seeing the hefty bills, that are not worth the care he is being given. I do not reccomend this nursing home and if you are thinking of Elms Haven for your loved one, ask them to show you the outcome of the State Inspection Deficiencies. Or look for yourself at http://local-nursing-homes.com/nursinghome.cfm?detail=inspections&home=elms-haven-care-and-rehabilitation-center&provNum=065238. They are due for another inspection this year and I am anxious to review the results.
