    This nursing home is not the place to send someone you really care about. The place is really understaffed, the food is mediocre and the general air about the facility is gloomy and depressing. The beds are very old and they do not enough bath towels for the patients to use. Padding's for the incontinent patients are rationed so that patients are in a pool of their own urine before the overworked CNAs' finally get to them. There are frequent outbreaks of skin related infections. Just overall poor nursing care.

    My mom was transferred to this facility diagnosed with Alzheimers. The staff in the facility set up meetings to discuss my mom's needs and they were really good about responding to our request. There are many caring CNA, nurses and other office staff people in this rehab. I have seen nurses spend a long time trying to adminster medication to reluctant patients with patience and care and most of the CNA's have number of patience but most are very patient and caring. These CNA's deserve less beds and more encouragement as they care for your loved ones all the time. The facility functions well on the weekdays but it seems that there is not enough staff on the weekends. I had the most trouble with the physical therapy since they don't really do any physical therapy with Alzheimer patience eventhough my mom was immobile and she could not walk. I guess they only work with patients who follow command but patients who can not communicate should also get some type of physical therapy. We had to put in several request to make sure she is getting physical therapy of some kind. so we would not lose her mobility completely. As with any other business, you have to follow through with your loved ones needs and make sure the staff understands your patient's specific needs such as nutrition or sleeping habits etc, they are good about making changes and accomodating your request. Some of the changes are slow to be made but they are eventually made. Overall, we are very satisfied with this facility.

    My father spent 9 days in Sherman Oaks Rehab before passing away. The staff at SO Rehab was not nearly as considerate or as trained as the staff at the rehab hospital where he had been prior to moving to SO Rehab. I could list everything but the final point I would like to make was that my father was a skilled nursing patient with at least 80 days left on Medicare prior to his death. The day before he died, the business office called to inform me about costs that would incur without bothering to check that my father required skilled nursing (he had a G tube) and that he was fully covered by Medicare for those golden 100 days. When the rep. was informed he "apologized" and then promptly sent out a bill for the following month, which shouldn't have happened as my dad was fully covered by Medicare. My dad died the next morning and the bill arrived today. Ironically, I live in Boston and they called my on my 617 cell number on the very day we were in lockdown.

    SO Health and Rehab took my mother in and they did their best to accommodate her needs. We dealt with the most part hard working CNA's like Sonia, Desiree LVN's like Bell and Joyce. We spent 14 hours daily with mom assisting in her care. Mom was obese with 100lbs weight gain secondary to tumor in kidney and clott in the liver causing a type of cirrhosis. We made sure she was turned every 2-4 hours. We fed her since it took her time to swallow. We helped turn her since it required 2-3 people to move her up in bed. What I can say is that you need to make sure that the CNA who is caring for your loved one must have a balanced assignment. If not, your loved one will suffer the delay in care. You can speak with the head nurse or management to make sure this happens more often than not. What I found was that there are biased assignments in nursing and I spoke to Araceli who then passed on the message to nursing and this was not much of an issue thereafter. I also found that mom was scheduled for daily morning therapy that was not initiated until about 1 month after she was there. Make sure your loved one gets the care they are approved for- spoke with finance and made sure mom got her therapy. Also, we had a couple CNAs who did not have much compassion. They looked at her in disgust and make inappropriate remarks- we understood the weight burden this caused on the staff, hence why we tried our best to help out. These problems were few and the compassion of the other staff totally made up for the few rotten apples. We had to place notes above the head of the bed to inform the new nurse of mom's specific care needs. What I can say is that family and staff need to work together to care for the patient. You cant expect excellent care if you walk away and just leave your loved one in a nursing home. Be aware of the level of care your loved one will need and always be willing to help out. Although we encountered the bumps in our journey, I was very happy with the care mom received overall. We were actively involved in all the medications mom took and got proper authorization from medical to continue the medication. Thanks to Jackie the nursing supervisor at night. We were approved for 4 hours after time of death to wait for all her kids to drive in to pray the rosary.Thanks to Dr Farouqui for giving us all a chance to spend the extra months with mom. He ordered the appropriate labs, xrays, medications to extend her life and was instrumental in mom having her 64th Bday, her last Thanksgiving, Christmas, and New year. In short, SO rehab worked with us and gave us the gift of life each day mom was there. We had plenty of time to grieve and come to terms with her illness. The nursing home is located one block from Sherman Oaks Park. This was the same park mom would play with us as children. We took her daily to the park so she would get some air and see other children play. It was good exercise for us too.

    Sherman Oaks Rehab was not the first choice for my mother. The facility we wanted had no vacancies. However, It turned out to be a blessing because I am very happy here as is Mom. Nursing homes are never perfect but I found that Sherman Oaks Rehab was willing to work with me in making Mom comfortable and well cared for. I spend 6 to 8 hours daily and I see how the employees treat those residents that have no outside visitors or family. They are loving and caring with these people, as well. I have called many times at all hours and they are wonderful in helping me with any concerns I might have. Most importantly, they are constantly striving to make this the best facility possible. I feel like they are an extended family.

    One of my closest family members suffered a severe stroke about 14 months ago and has been living full-time at the S O Rehab Center for the last 13 months. Although it is always best and recommended to advocate for a loved one, it is ultimately beneficial to also be a frequent visitor, a volunteer and active member of the family council committee at the Rehab. Beyond this, I have found the S O Rehab to be for the most part a concerned facility for the care and treatment of their patients, how it is run and how the facility is kept clean and up to high standards. They have been attentive to suggestions, concerns and whatever help we have required. I truly bless the CNAs on the front lines who do such a wonderful job for my loved one and would recommend this facility to anyone in need of its care. PW

    My mother basically died here after a month and contracting pneumonia. Someone in the facility stole one of her bank checks and used it to subscribe to cable. They only had one phone at the front desk and when I tried to talk to my mother long distance to that phone she could not hear me. The social worker was very rude and basically I have always been suspicious as to whether they contributed to her death.
