Review of Santa Cruz Post Acute

• I have twice been a patient at Kindred Nursing and Transitional Care, Santa Cruz following total hip replacement surgeries to both my left and right hips. My first stay was in December 2012 and the second was in June 2013. Following each surgery, I had numerous complications/issues and was told by my case manager at Sutter Maternity & Surgery Center that I needed to go to Transitional Care facility because I was in no condition to take care of my self at home. I have had five surgeries on my left knee and have never felt as weak helpless as I did when I arrived at Kindred the first time in December 2012. I was a bit apprehensive because I had never stayed at a Transitional Care facility and did not know what to expect, but after the first day, I knew Kindred was where I needed to be. I had a couple of issues that were quickly resolved and both my stays at Kindred were productive and pleasant.

The staff that took care of me both of my stays was exceptional; from the RN’s, Occupational & Physical Therapists to the CNAs. The Occupational Therapists made sure that I was prepared to go home and be safe and comfortable with my limited abilities. The Physical Therapists showed me many valuable exercises that I could do at home with no special equipment and each time I went home with an exercise program tailored to my abilities and fitness levels. From the moment I arrived until I was discharged, the RN’s, and the CNAs took care of my every need and made my stay as pleasant and comfortable as possible so I could solely focus on getting fit and mobile. When not in therapy, the Activities Director and her staff always had something going on. I particularly enjoyed the morning coffee session. In addition, Kindred has WIFI and Activities always had a lap top I could borrow so I could keep in touch with my friends.

Each time I came home, my HMO set me up with Visiting Nurse Association for at home Occupational and Physical Therapy. Both the Occupational and Physical Therapists sent by VNA to evaluate me were impressed with the training, conditioning and therapy I received at Kindred.

At the end of 2013, I am going to have a total knee replacement. If my case manager determines I need to stay at a Transitional Care facility following discharge, there is no doubt in my mind where I want to go and I hope that Kindred will have a bed available for me. I cannot imagine going anywhere else!
