    My family found this facility to be below par on many levels. Staffing is inadequate and most are not trained to support the level of care that patients they accept need. The management would differ but after observing this operation I would not allow anyone I know to be placed in this facility. With the exception of only a few nurses and CNA's the level of incompetence and lack of caring shown to patients is unbelievable. They do not follow proper procol for hygiene and cleaning of patients. Most do not follow handwashing and sanitizing portocols established to prevent the spread of disease and infections. The equipment and furnishing they provide the patients is in poor repair and not maintained. They do not respond to patients calls for assistance in a timely fashion whether it be basic or critical needs. Response to call lights and calls for help often exceed 1 hour or more. Most of the nursing staff do not recognized critical symptoms of distress in patients resulting in emergencies, infections and more serious complications to the patients. I could continue on with all the horrors I saw. I can only say that the near death experience of a loved one due to the lack of care provided was enough for me to warn others of the danger you are putting you loved ones in if you choose to place them in this facility. Please don't be fooled by the smiles and promises.

    My wife's dad stayed there for about 8 years. Overall, the care providers there did a good job, and he was happy there. The place was kept clean, and the nursing staff was friendly and attentive to his needs. However on December 3, 2013 my wife was notified by the care facility that her dad had suddenly passed away. She told them that we would be down the next morning to take care of things and they said that would be fine. Upon our arrival, we found that his room had been cleaned out and someone else had been moved in there, and all of his personal belongings had also been removed and put into cardboard boxes that were sitting on the floor in an office. His name tag on his room door had been also been replaced with someone else's name tag, and this whole experience really devastated my wife. She and her dad were very close, and she was his only family. The fact is that the management staff at the care facility knew that we were driving the 160 plus miles down there the next morning and chose not to wait and allow for her to personally remove her dad's personal effects. This was so disrespectful and upsetting to both of us. My wife was ready to cry and I was so angry that it took a lot for me to not lose my composure. My wife wanted to just sit in his room that he had called home for the last 8 years and take it all in as to how his last morning on this Earth started out. The fact that the care facility chose to fill his bed the very same afternoon that he passed away just makes us both very angry and disgusted. Is the money factor the bottom line for these places? You would think that since the facility had been paid a ridiculous amount of money each and every month for 8 years, that they would have had the dignity and compassion to wait the extra 12 hours or so before filling his bed. These greedy bastards should be ashamed of themselves for causing such a terrible ending to an otherwise decent experience. We will definitely not forget it.

    We recently had an experience with Amberwood Gardens and found the care and environment excellent. My father in law was transferred to Amberwood from Kaiser hospital and we were anxious about what sort of care would be provided. Our overall impression could not have been better. The facility was clean and well kept up. They could provide exercise and rehabilitation services. The most important thing was the staff. We found them to be excellent. Down to the last person we had contact with the mood was always respectful, upbeat and helpful. The treatment of my father in law was very professional and extremely caring. We did not feel like he was just another person in the home but mattered to all who came in contact with him. They had an outstanding attitude and made us feel like we had found the absolute best place for his condition. I would not hesitate to recommend this nursing home to anyone.

    This place is a JOKE. When people saying "health is a business" this is a prime example. This place is runned off of contracts from other health business's.

    Had a family member here who never recieved any rehabilitation, although they call themselves a rehab center. They are more concerned on how it looks thank what it actually does for people. I would compare this place to a cheap motel, because even at a cheap motel can you actually get the same help from the front desk than these folks.

    They are more quick to call you about how much money you owe them for the services they didint provide than the actual care of your family member. If you care about you family don't bring them here. Just an expensive cheap motel.

    After a bad experience at a different nursing facility for a family member I was distressed to find out my son would have to go to one for some rehab after a major surgery. He was referred to this one by the hospital he was in and has done very well, I cannot praise the Administrators enough and the nursing staff are so caring and compassionate. I am so impressed with all of the activities they have and the fact that they take the time to get everyone up, dressed and as involved as possible,they have an outdoor garden area for patient use, everything is clean and it doesn't smell like a nursing home.they have a morning coffee social, movies, music, a lot of interaction for the people who are there,.thank you for running the kind of a place that I can feel good about having my son be there..

    I am an ICU Nurse at one of the local hospitals. I am well versed with the ins and outs of Skilled Nursing Facilities. This facility is a "trusted" facility for professionals at the local hospital accross the street. There are two of us nurses that have had our parent rehabilitaite here. These online reviews do not take many things into consideration. These elderly folks and their kids are mostly not tech savy therefore are unable to give reviews like some of us can. The online reviews also do not take into consideration that this place is a revolving door of patients coming and going. They can discharge up to 30+ patients monthly only to bring in just as many. My mother has been here on more than one occassion and she will return if more care is needed. I have a trained eye to see how skilled nurses are when caring for patients and these nurses at this facility are the best you can get in a facility. The rehab she recieved in house kept her on her toes and brought her back to the brink of loosing her mobility. It is an added bonus that this place has an affluent character. It is not only clean, it is polished with care. Tour these places and compare for yourself. There is a facility near Plum Tree that I wouldn't take my dog to because of the care patients recieved there. These patients came back to hospital with horror stories of how incompetent staff was. Please, tour these facilities and ask nurses and doctors for their help in selecting a facility. They will point you in the direction Plum Tree. Thank you to the staff their for all their hard work and caring nature.

    Amberwood is one of the nicest facilities i have ever seen. My mother was in a couple different facilities before i found this one and this is heaven in comparison. The staff shows genuine care for every resident and are very helpful in making my mother comfortable and happy. Many thanks for this facility, stop by and see for yourself!

    My father injured his back and needed recovery time in rehab. Sunnyvale Health Care Center was recommended to me as a rehab center to consider. The facility is about 30 minutes from where I live, so I decided to go online and examine SVHCC's website.

    All looked acceptable on their web site but I was let down when I saw their facility in person. First of all, the facility is very old and has had very minimal updating done inside and out. The rooms for the patients in rehab are quite small, very dated looking and uninviting. I was appalled at the tiny bathrooms that 4 patients have to share; there was barely no room for a wheelchair to fit inside the tiny door opening. The sink in the bathroom that I inspected was most unsightly; old, stained and the faucet was dripping hot water. Not too mention the bathroom had a stale, mildew like odor.

    I also want to mention that there was a rather hostile lady, a resident I was told, who was being led by a nurse down the corridor. This lady was spontaneously shouting profanities and appeared violent. She didn't look as old as some of the other residents, but her bellicose temperament was most alarming to say the least.

    The final red flag raised was witnessing employees smoking on the campus of SVHCC. I was shocked to learn that smoking is permissible for residents and employees! My father was never a smoker and is very sensitive to such irritants.

    It pays to do some research and investigating before making any rash decisions.

    I would not recommend this facility as acceptable.

    The staff is very good. The problem is mixing rudd patiants with short stay non rudd . minded people. Respect is the problem here. Some paitiants treat caregiver down right nasty, very demanding . My mato you get what you give. oh and the cook staff should lay off late nite partys. give me a break. change jobs PLEASE..

    My husband has been a resident for several months. He has total mental capacity and is there due to his general frailty. Since he has been put on Medi-Cal he has been moved several times, each room worse than the last. He is not a complainer, and I think for that reason, the staff thinks he's happy. He is now in an area which has an odor (hard to explain) and a lot of noise from other residents. His bathroom has a mildew or moldy smell which cannot be removed by a simple housekeeping once over. There is an underlying cause. In recent weeks a lot of remodeling of the staff's quarters has been done, but nothing has been done to improve the quality of life for the people who don't get to go home at night. As far as the noise and shouting from other residents is concerned, I know they have their problems, but couldn't there be areas for residents who don't have those problems? I have approached a couple of S'Vale's staff with my issues, but so far nothing at all has been done to remedy the situation. I am embarrassed when friends come to visit him because of the odor. This is his home now, and he should be able to look forward to having people come to visit. I am disappointed. We were led to believe that his long term care would be a pleasant experience.

    I had such high expectations for my mother here and it seems as if they gave up much too fast. No follow through with the promised care from our initial meeting on what was going to happen for the well being. Staff ignore alot of the call lights as I have had to go get them myself to tell them someone is in need of help.

    Disappointed that this is the type of care offered for the elderly.

    No wonder they have a federal 2 star rating.

    This facility is caring nurturing and helpful in dealing with all of the paperwork, working with the Admissions and with Azita personally, she knows her job, and works very hard to make sure you get top quality care, and really advocates for her patients and their families. There is a respect and knowledge that I have not seen at other facilities before.

    We are all so lucky to have her on our team.

    Valley House is a pretty special place. The rooms are big and have been remodeled. They do their best to meet the individual food choices of residents and have a very upbeat and encouraging staff , especially in rehab. The outdoor areas are full of flowers and quiet sitting areas and there is parking! Most importantly, there are a lot of people who work there who actually care about the residents and many of the residents have personal challenges, physical or mental , that sometimes make it hard to care for them 24 hours a day. There isn'ta better rehab center in the valley.

    This is an excellent facility. The speech therapists, dieticians, and nursing staff are all very, very friendly and knowledgable. They're attentive and caring. The administrative staff are very on top of all the paperwork. The cleaning and assistance staff are gentle with the elderly. This is a trustworthy place. I would only suggest that you supplement food given with that cooked at home, if you really want someone to gain weight. The food isn't "great" but it is passable as food…

    I give the CNA's and PT's five stars A++++; very dedicated, caring, gentle and sweet natured. The RN's are a whole other another story. They won't listen to the patient nor will they listen to the loved one's of the patient. They overmedicated my Mom with a strong narcotic pain med and sedative that caused her to have heart palpitations, delerium and severe constipation. I really hate this place and still have nighmares about my beautiful Mom being in this hellhole.

    My mom is a retired Secretary who has a small steady income from Social Security and a small amount put away from dad's retirement savings. She can't pay co-pays for her rehabilitation stay. Medicare pays the first 20 days so our goal was to have her out by the 21st day. The boys in the therapy department knew our goal and worked with her for over an hour 2x a day to get her stronger. They were patient and encouraging. We were able to check out before our set goal time. Thank you to the staff at Woodlands especially the Therapy guys!

    We passed by Woodlands all the time never knew it was there because it's hidden. I had a very good feeling about this place when we arrived to tour for my mother. Before we reached the Lobby, we saw what turned out to be a physical therapist walking and encouraging a patient in the garden area. I was tuned into the gentle but firm way he was encouraging the patient; "You can do it, Come on, just a few more feet". After touring other places near by, I was relieved that Father was ready to make his choice for Mothers Rehab, "Woodlands". Mother completed therapy in less than a months time. They were even able to accomodate our late visits as long as we kept our noise level down. Woodlands should change it's name to "Wonderful" because that's exactly the kind of experience you will have there. Many thanks to the men in therapy room and to the CNA's who were very instrumental in Mothers recovery. God Bless You all.

    My father was recently discharged from Vista Manor Nursing Center. I would like to thank the facility staff, especially the rehab staff who worked very hard with my father. The building is very clean and my father felt very comfortable there. I highly recommend this skilled nursing facility to everyone who are looking for excellent service.

    I was released from Woodlands a few weeks ago. I was depressed going to a SNF but after my initial therapy session I knew there was light at the end of the tunnel. The men in the therapy Dept. were fantastic. I was in and out in no time. Nursing was good and the rooms were nicer than the hospital. Thank you Woodlands for helping to restore my Independence.

    There is a problem at your facility with the telephone used to contact patients. It appears as if the system is either outdated and defective. I have made calls to my relative there and the connection is horrible. One cannot exchange conversation without interference. The end result is not being able to speak to the patient at all. It would be beneficial if this problem could be corrected. The calls I make to your facility are long distance calls so I am paying for nothing.

    My grandpa was in skilled nursing rehab at The Forum for a month healing from back surgery. The place is newer compared to most nursing homes and looked pretty decent. The Forum is nestled in a very lovely park like setting in Cupertino, California. The nurses, CNA's and physical therapists were great. The only complaint my grandpa had, including myself, was the loud music that could be heard, and often, from the dining room. My grandpa hates accordion music (I do even more) and one evening it was real loud so I had to shut the door in his room and then it got too hot in the room:( There were some days when there was loud singing (sounded like a drunk) pounding away at the piano! It's too bad the facility couldn't have been designed with the dining room far from the patients rooms. Aside from those pesky noise issues The Forum treated my grandpa well.
