This facility currently has poor quality care. Our hospital receives patients from this nursing home in poor condition, complaining of poor care, lack of social activities, and most of all poor manipulative management. If you can send your loved one somewhere else, you should do so.

Thank you for the review. My uncle got transferred there last night from tri-city convalescent, previously coast plaza medical, this facility is extremely smell, hot and small. Staffing there have no idea where my uncle room is nor when he got admitted. This is the reason I decided to Google and found tons of discrepancies. Btw, previous name is sunrise care&rehab-intercommunity and they got massive discrepancies under that name as well.
