The physical therapy is very good…but, the facility as a whole needs some improvement. There needs to be clear and concise communication between the chart nurses and the nurses attending the patients. A patient was told that they are not suppose to use straws when drinking anything. The dietitian said everyone ought to know that. A nurse attending that same patient had a straw on the table, ready to use it because they were not told that patient does not use straws; The patient requested only female nurses to attend to their personal needs and not one (1) but (2) two times was a male nurse assigned. The physical therapist was very certain that everyone in the facility ought to know that the brace a patient wears is only for sitting or walking, not while they are in bed. The patient had the brace on all through the evening until they were checked again. Their skin was irritating red and it was so tight it left markings on the skin and it was difficult to remove. The nurse at the time admitted they had no knowledge they were suppose to take it off while she was in bed. They have different nurses attending the same patient so it makes it difficult for a nurse to know what to expect and what proper treatment the patient really needs….They have feeders but, they demonstrate no patience to feed someone who may need a little more than just putting a spoon in their mouths. When a patient usually rings a bell, no one shows up until maybe 10 to 12 minutes later…sometimes more….They say they check patients every two (2) hours, but, a patient that cannot fen for themselves needs to be checked more often than that….patients are found very soiled more than once and need to call for a nurse to change them.

The facility has gone through several management changes since this review, which has improved communication between the nurse and therapy staff. Associates have received customer service inservices on a continual basis in order to meet and exceed the expectations of the residents we serve.
