This is a full service nursing facility, not an assisted living facility. Most residents are long term and this will likely be the last facility they will live in. If that is what you need, this place is GREAT!! My mother was here for over 10 years until she recently passed away. She was almost completely paralyzed and could not do anything for herself. The staff and nurses at this facility are very loving and caring.
This facility is in a lovely and quaint residential neighbor hood. The grounds have a large lawn where you can sit under a shaded tree or in the sun. It's 2 blocks from old town Pleasanton which is a step back in time and a great place to take your family member when you visit to get some fresh air, food and a change of scenery. Nice parks within 2 blocks.
If you have someone who is fully ambulatory and energetic who is only looking for short-term rehabilitation (not our situation), I can't comment on how this facility compares to others. If you expect resort-like recreation and food, you will be disappointed. If price is no object for you and you are looking for a 5 star hotel experience, then keep looking. But if you are looking for a place where your family member can live out their last years and have a staff that genuinely cares for it's residents, this is a good choice.

I have lived at Windsor Post-Acute Care Center of Hayward for almost 4 years now and have yet to really have any major complaints.
Lets start with housekeeping as that was the first thing I noticed when I came through the door for the first time: as an unusually young man for these facilities of almost 34, I have been in far more than my fair share of these types of facilities all over the San Francisco Bay Area, and most commonly, I have found that when you come through the door, the first scent that hits you is dirty laundry and urine and bowel movement soaked garbage. Not here - when I first walked through the door, and every day since then, I most commonly smell a CLEAN smell. They're not at all afraid to use cleaning products here!
Nursing (RN's/LVN's/CNA's): Unlike a great deal of the other Residents here, I understand that the Nursing Staff tend to get busy and I'm not their only patient they have to tend to. When I need something and they're not there to help me IMMEDIATELY, I don't get upset. Then again, I'm not left waiting very long anyway - they always come to me and tell me what's going on and give me a general "heads up" as to when they can come help me if they're going to be busy for an extended time. Of course, if it's urgent, they'll help me right away.
Dietary: I'm known as a picky, picky, picky eater. This (and several other facilities, I'm sure,) have pre-planned menus. That said, our Dietary Staff always has a backup plan in place for myself and anyone else who may not like whatever is on the menu for any given day. There has NEVER been a day where I received food I didn't like and I had to eat it.
Laundry: Most personal clothing is washed, dried and returned within 1-2 days. (Shh!) The only frustration is occasionally, you may end up with the wrong person's clothing. No big deal though, just return it to the Laundry Department and they'll re-deliver correctly - they're great about it.
I can't say much else about the Staff - The Administration Staff and the Maintenance Supervisor are the best in the business. They bend over backward, forward, and sideways, if needed, to make sure your needs are met and your stay at Windsor Post-Acute Care Center of Hayward is as wonderful and comfortable as possible.

I stayed there four days after abdominal surgery to regain enough strength to handle the many stairs to and in my home. I found it clean (no smells, frequent mopping of floors), but the food was terrible. I am a vegetarian, bad the kitchen acted as if they had never heard of such a thing. One dinner entree was one whole wheat hamburger bun with a 3/8" slab of peanut butter. That was it. And their staff only got my medications to me correctly and on time twice out of 20+ times. I have a complicated schedule of medications that I must take at 5 different times each day. If I didn't know that by heart and look at the pills they gave me and question them, I would have gotten the wrong meds almost every time. Their physical and occupational therapists were good and I had myself wheeled down to their room twice a day, once for a session and once just to work out on the equipment. I'm glad I did. That's what enabled me to leave after only four days. I couldn't handle the "food" any more. The CNAs were very nice, in general, and a few were real gems, kind and caring. Some of the RNs had an attitude problem, but others were nice and competent. I survived and got strong enough to get home pretty quickly, but that was my doing more than theirs.

The physical therapy staff were fantastic. I could not have done anything without them. The staff was wonderful and they tried to be very helpful. If I ever have another problem I would definately come here again.

This place is not the place you want to bring your loved ones. Both my grandmother and grandfather were brought here and both died shortly after being brought here. Now, originally each were taken to Eden Medical center in Castro valley, for illnesses that were not terminal. For example, my grandmother has gall stones which were removed at Eden and she was released to Windsor gardens a week later. However, at Windsor gardens, she was said to be recovering well, but 3 weeks later became septic. Windsor had her taken back to Eden were she recovered again and was released to Windsor, and then somehow got Bilateral Pneumonia. She then passed away at Eden. The same things kept happening with my grandfather, he always seemed okay, like he was getting better, but after a week or so of being at Windsor, he would get sick again. He passed away after all of they back and forth as well. One day, when I went to see my grandma, I was observing the nurses during lunch time and I almost couldn't hold my tears. I watched 2 of the female nurses, literally pick on a woman with Alzheimer's who did not speak english. They were making fun of her and taking her doll from her and making her very upset right in front of me. Then, one began to feed a patient who seemed to be paralyzed and was shoveling the most disgusting mush into her mouth over and over again. She wasn't even paying attention just shoving food in her mouth while the women slowly gummed at the mush. The nurse had the food spilling out the the woman's mouth and would barely clean it off her face. All she would do is use the spoon to scoop it up and then shove it back into her mouth. There was another male nurse who came over said "hey, that's my feeder, that's my feeder", I couldn't believe it. She is a patient not an animal!. I was getting so upset watching these nurses treat these patients like animals in a zoo. I hope to never have to bring any other family members to this place. It has been a horrible experience for my family in the last year.

I am not satisfied with the care given to my grandmother. Every time I visit which is about two and three days consecutively, she's full of urine and bowel or she is not being turned as often as she should. Every time you turn around CNAs are complaining about how the center doesn't have basic necessities such as lotion soap and spray for cleaning residents. In addition diapers are always coming up missing. Plus, when we volunteer to change her the CNAs do not help or have nasty attitudes about changing my grandmother.

unfortunately this place is less then desirable. some but few staff are great. there have been meetings with directors and staff regarding care and no change. also they must be in cohoots with st rose hospital and are getting some kind of a kick back because they REFUSE to take pts to her regular specialists as directed by drs and st rose is for life or death emergencies only. that was also discussed in meetings with directors and never upheld. im constantly ignored at the front desk as are other visitors and vendors. there could be someone sitting there on their phone or chatting and they wont stop what they are doing to address any needs or questions you may have. things are constantly stolen from pts. they are never on top of changing dressings for their pts. i even have one recorded where a nurse is telling a pts to wait to be changed and that they had things to do! what kind of care is that?! its unfortunate that ppl who cant afford 5* treatment need to settle for horrible care, knowing that if that were their family member they would go about things differently. its a shame.

Do Not place your loved one in this facility. They do not monitor dementia / Alzheimer's patients whom yell, moan and scream. They ignore the loved one whom moans, yells and screams. This is not proper treatment. If the only way your loved one can communicate is by yelling and screaming, they do not receive help for their discomfort. If you have a loved one who does not yell and scream but is residing near a patient who yells, moans and screams, your loved one will get no rest, no sleep and no relief. I have filed several complaints at the facility and I am now pursuing other avenues to file complaints.

We went in to take a look at the facility to see if we are going to put our dad in there for short term care. We were shocked. First, on the way in, a patient in a wheelchair was stuck in the door which is not automatic. Two nurses were right there and other nursing staff walked by and none of them helped that lady. My elderly father went to help her. Now that is sad! After waiting for about 10 minutes, the admissions person finally came and gave us a tour. The facility is not good. They have 3 bedroom rooms; the shower facilities actually have 6 showers in them - meaning, patients have to take a shower together in one room; but that probably would not happen because they used the shower facilities as storage rooms for stuff (literally - JUNK!). My parents were horrified! I was really angry. The hallways had stuff standing around causing safety hazards. In some areas, it smelled like urine. This whole experience was really bad. If I could give this facility less than one star, I would give it negative 3. I hope the state ombudsman does an anonymous tour there and pretends to be a visitor asking for a tour.

We went to the facility to get a tour to see if we should put our dad in there for short-term care and I can say, I will not put my dad in there. They have 2 - 3 room beds, the site is loud, people in wheel chairs were lined up against the walls in the hallways and nobody was paying any attention to them. They do have activities there. They were planning a luau for the residents and they had bingo going on, but if the would put that effort into actual patient care rather than activities, this place may even deserve three stars.

My father has been here for 3 years.. it has been good, but has now turnin n to some awesomeness.. since 2012.. staff are movin and on it.. get to it. workin like they mean it.. happy my father his in his last days. The best healthcare to the needed people within their entitled medicare benefit. I have no complain against the Alameda Healthcare & Wellness Center. They have done a lot to care for the residents in the facility. My in-laws have been with the center for almost a month. I will rate the services they provided excellent.

My sister suffered a stroke and Kaiser put her in this facility fot therapy. The therapy has been okay, but other than that you receive very little assistance from the aides and RN'S. Since she cannot use her right hand they do not provide assistance at mealtime.
She pushes her button to go the bathroom and .the wait has been 1 to 1 1/2 hours before she receives assistance.

This place was OK a while ago, but they are cutting on staff and spending. It is getting worse day by day. The administrators tell newcomers that they have one Nurse's Assistant (CNA) per 6 patients during the day, but that is not true. In actuality each CNA has 8-12 patients to take care of, which is impossible, so patients and long term residents are suffering. And the CNA's are overworked. They are skipping on showers and cleaning. The food quality has deteriorated greatly. The food tastes terrible, so most patients are not getting adequate nutrition. Kitchen is almost late every day in serving lunch. Some patients do not even get their diapers checked or changed as they should ever two hours. The place is stuffy and smells bad. The owners are pocketing a lot of money and not spending what they should to take care of people's loved ones. If you have a patient here, please check on them every day and maybe find a better place to move them to.

I am deeply saddened that the Management at Windsor Park, 2400 Parkside Drive in Fremont, did not acknowledge my mother’s passing away after a 2 month stay at their facility. After all, this was her home for the last two months of her life and she passed away there. Being a facility where this happens all the time, I cannot believe that there are no procedures in place to deal with the death of residents.

my father is at this facility and having been there every day to visit with him i feel i can give a honest review. I believe the patient care is very good and it seems to me that the staff really care about their patients. Everyone has been so nice and easy to deal with at this troubleing time for my dad and my family. The food looks and smells real good and it must taste real good as my dad is eating it everyday and he is very picky. the only somewhat negative thing i have to say is that the facility itself needs to be upgraded. the patient rooms are pretty drab and could use some paint and maybe some new flooring, new curtains, drapes. It would brighten up the patients rooms and therefore make them happier… I would and will recommend this facility if anyone ever asks about a healthcare center. thanks again for taking such good care of my dad.. jude (maggie) andrews 3/31/2012

The nursing staff work hard and are dedicated and caring but have too many patients to care for. So during meal times they cannot deal with patients who need immediate attention. Delays are inevitable. It is also hard for them to give any extra attention to a patient with special needs.
The rehabilitation staff are very good and work well with the patients.
The food served is wholesome but the quantity seems to be too much for most of the patients resulting in a lot of waste.
The rooms with 3 patients in them are cramped and hard to move about in. They appear dingy and do not get much sunlight.
The administrative staff are helpful and generally attentive so I was surprised that they did not bother to send the family any letter of condolence or sympathy after my mother passed away there after a 2 month stay.
