    If at all possible, DO NOT EVEN CONSIDER PUTTING YOUR FAMILY MEMBER IN THIS HOME. The experience today was not only upsetting but absolutely disgusting. From the front desk to the administrator I could not find one worker who was friendly or had a smile on their face. I was with a business person to help deliver a resident's van at the facility and we were promptly visited by the administrator telling us we could not store the van there. I asked him several times to explain the reason why because the van was NOT being stored. He would not answer me and would only say it was company policy. We tried to talk with him and he threatened to have the business person escorted out of the building. At the same time he was harassing the resident telling him to get the van out of there. He gave him no time to handle the situation and upset the resident. This administrator was not professional and did not care about upsetting this resident. He should not be in the position he is in.

    My loved one was transferred here yesterday. It took 5 hours just to get a glass of water. They left her in her own feces for over an hour and now she has the worst rash. The place looks so run down once you get to the wing. Due to the long waits she has to endure unless someone can walk out to the main room (which she cannot do) and the above mentioned reasons, we are looking at moving her out of this horrible place tomorrow. They are delaying things by refusing to send records, so we got a copy ourselves and will be hand delivering them to the other places. Please keep your loved ones away from this place!

    I am basing this review on recent events as well as past ones. My brother was a resident who died there in September. Since I live several states away I had asked the social worker to mail my brother's personal belongings to me for which I would send the postage fees. She assured me that she had all of his things safe with her and would do so……after many unreturned phone calls to her and he supervisor I finally emailed the director which also received no response. Well, today I got a phone call from the NEW director of social services to tell me she had found my brothers wallet in her desk and will mail it to me right away. Obviously. the former employees managed to steal a cell phone, a ring, eyeglasses and a brand new tablet and had reason for not returning my calls. Thank goodness they have hired someone honest to work there!!

    DO NOT leave your loved one here!!!! My loved one was pulled out of this facility due to abuse! I won't go into details but she wound up in the hospital and a social worker immediately moved her out of that hell hole. This place should be shut down. I am sick to my stomach to think what abuse is going on that doesn't get caught. If I could rate this place a zero I would!

    This is a beautiful facility that is changing in amazing ways. They recently became a 5 star facility in Quality Measures by Medicare. The staff is accommodating and do their best to provide the best patient care possible. The food is excellent and their therapy team is top of the line. Overall it is a great place that I would be comfortable entrusting my families care to them.

    zero. do not take your family members here. This is the worst place I have been to. When you first get a tour the place is clean and they have places to sit but after you are admitted they just don't take care of you and worse than that, they just don't seem to care at all. The family gets a tour and agrees to let their family member stay, but the sick family member gets no tour, they go over nothing with them. They just make them wait and wait, without any explanation. The staff take more time explaining why they can't do something or they say they will get whoever can instead of just doing it. We usually had to ask multiple times for anything.

    We had to remove my father. When he was admitted he was there for rehabilitation after having surgery and being in the hospital for 5 weeks. We kept getting excuses as to why he didn't start rehabilitation. Not enough staff, waiting for orders. They only shower them 2 times per week and on the 4th day there he still had no shower. We missed one day of visiting and when we got there the following day my father could not speak and didn't know where he was. They had him sitting in a chair facing traffic on Craycroft. When I asked them what is going on, he can't speak and he was ok 2 days ago.. the staff had no answer. The doctor wasn't in and hadn't seen him. We asked that we get him to the hospital right away, that something was wrong. They wanted us to wait for the doctor to get a release. We waited for a couple of hours and nothing. Finally we said we are leaving for the emergency room now. They made us sign something but wouldn't transport him. We did ourselves and after tests found out my father had a seizure from scar tissue that was growing in his brain. My father had another surgery and after recovery he told us he knew all that was going on .. he just could not communicate and the staff at Pueblo didn't pay attention. He remembered the minute he had his seizure and everything changed but none of them noticed. He said he tried to talk but couldn't say anything that anyone could understand, but nobody noticed that he talked perfectly and laughed the day before and could not speak the next day. How does this happen?

    Very very bad experience. I was admitted because I had broken my femur, had surgery and needed nursing care. The day after my arrival I called for help to go to the bathroom. The tech that came to assist me failed to support my left leg while helping me get out of bed which resulted in my leg falling off the bed and causing horrific pain for days. I was in so much pain that I could not get physical therapy which set me back 5 days. The sad thing about it was that several of the tech's were aware of his poor job performance this was not the first time he had mistreated patients there. The facility never put what had happened in my records. They only stated that I had extreme pain therefore I had to get X-rays.

    Three days before I was released I had physical therapy and the therapist asked me what concerns do I have when I arrive at home. I told him my front door has a 2 or 3 inch threshold and I'm worried about getting in with the walker. The therapist brought over a constructed steps with railing and told me to hop on the first step. I was scared that I couldn't hop onto the step with one leg. He said, "Don't worry you won't fall, I'll hold you with the gait belt" I did hop up the first then the second then the 3rd and finally the 4th step with him holding onto to me with the gait belt and all this time I was supporting myself with the hand rails and needless to say my bad leg was just hanging there. He had me hop down the 4 steps and again he had me hop up and down. 8 times up and 8 times down. A little bit of over kill, don't you think? All of this so I could get into my house with a 2 inch threshold. This therapist just happens to be the head of physical therapy. And then that evening my left shoulder began to hurt and hurt a lot! I called the tech and told him what had happened to me during therapy and to note it in my file, and he told me he would do that. Shortly afterwards he came back with a small container of aspercream and I rubbed it into my arm and shoulder. This took place 17 days ago and I am still in very much pain and waiting on x-ray results.

    The food was horrible, my room mate was lactose intolerant and they kept bringing her food with lactose as a matter a fact they would add extra cheese on some of her meals. They had said that we could always get the alternate meals, but they never gave us that choice.

    The toilet had urine dried outside the bowl for at least 5 days and when I brought to their attention, the custodian couldn't see what I was talking about. I witnessed a tech clean feces on the hand rail in the bathroom after I brought it to her attention with a disposable cleaning towel instead of using a desinfected towel.

    One morning my roommate and I got yelled at in front of all the patients in the dining room for arriving at 8:00 am instead of 7:30 am for breakfast.. No one had told us that we had to be on time for breakfast or they would not serve us. She yelled, "You are late, you can't eat in here you have to go back to your rooms, we are not serving you!" This was very embarrassing and humiliating. Oh she tried to make it better by pulling me back into the dining room on my wheelchair and seated us and allowed us to order breakfast. My room mate and I were very upset and had no plans on returning any time soon. Well, about 6 days later we decided to give them the benefit of the doubt. We arrived for breakfast at 7:30 am sharp and unfortunately, we did not get served till 8:00 am and our food was served after 8:00 am. All of the fuss over being on time was for nothing, it didn't matter because they were still serving after 8:00 am. The cafeteria manager just wanted to show to everyone that she was the boss.

    Most of the technicians were very helpful and had empathy for us, but their were a few that had no business working there. My room mate and I were very aware of the technicians that did not like doing their jobs. Maybe it would be in the nursing homes best interest to continuously evaluate their employees because this type of work is obviously not for everyone!

    I am emotionally disturbed and very frightened of experiencing more pain because of how the technician failed to support my leg and also how the physical therapist put me through a very strenuous exercise that was totally uncalled for. I bet he wouldn't put his mother thru an exercise like that! Thank God I was only there for 14 days.

    I suggest if you have a loved one in a nursing home plan on being there almost 24/7. Because you have no idea what goes on while they are alone.

    Undertrained, incompetent staff.

    Unintelligent, uncompassionate, egotistical nurses.

    Poor quality of care.

    Good luck reaching the director of nursing!

    Promises to resolve problems are broken.

    Maybe this would be a good place if you bring your own doctor in as well as a private duty nurse.

    Otherwise, nothing good to say except about the food.

    When my daughter arrived at Life Care after a prolonged hospitalization and the beginning of a promising PT evaluation She was beginning to walk and assume other self-care responsibilities. After 3 weeks there sh as no longer able to walk or stand unassisted. The food was very poor. We more often than not brought her food on a daily basis. They didn't seem to understand or were unable to provide a low sodium diet. The staff was pleasant but very overworked and unable to give prompt patient care. Often they were out of her medication. Bottom line…after 3 weeks she was back in the hospital for 2 more weeks. I gave it one star because it exists.

    I have had my loved one in this facility for almost seven years. It was known as Desert Life, when I first placed my loved one in the care of this skilled nursing facility. Then Kindred Care took over. Prior to Casas Adobes which is operated and owned by Ensigngroup from California. I never had to deal with the management not being on top of the CNA's that they hire. And allow the CNA's to take over and down out right lie about the lack of care they deliver to the residences there,

    Since they took over we as family members see the lack of hygiene care to several of the residence and when brought to the managements attention it goes in one ear and out the other. Since they have a new Director since May 2014 - He's joke. He tells family members what he thinks they want tho hear and dismisses legitimate issues brought to his attention. he does not walk his talk.

    WE have a loved one in this facility.Upon entering, we came face to face with a closed entry and entered directly into the dining room at meal time with many completely helpless people sitting around the tables. Was very depressing and seemed highly susseptable to germs from incomming guests. The facility did not appear professional at all, as someone sitting at a table had to give us directions. Where was a director?

    Catalina post acute Rehabilitation center is an unsafe environment that does not provide it's residents with quality care. Most of the staff inexperienced with extremely limited educations. I have seen nurses go from room to room giving eye drops, administering shots and working with IVs without washing there hands or putting on gloves. When I asked one to put on gloves he responded with"I don't need gloves". I have come my times to visit one of the long term care patients and found them hunched over unable to move apparently left that way for hours. I have walked in on patients left sitting I their own fecal matter, excessive spume all over their neck, dirty safety mitts that have not been changed in weeks. Residents are given minimal therapy at best and if the person is recovering from a stroke or brain injury that has left them unable to move or speak they get ignored unless there is a family member there to speak up for their loved ones needs. I would not recommend this place to anyone. Catalina is one of the worst care facilities I have ever had to enter into and would strongly advise against sending your loved ones there.

    I would give zero stars if that were possible. My mom was in for over three weeks, a most horrendous experience. Understaffed, uncaring, untruthful, unaware, uninterested and unavailable staff. Don't even consider this place. The rehab therapy is a joke. A few minutes on antiquated machines with mostly sitting around waiting your turn. Your loved would do better at home. Absolutely no care to speak of!

    Handmaker was recommended to me by two former employees, one an RN and the other a social worker. They worked there several years ago. Unfortunately the experience my aunt had there was horrifying. We eventually had to take her back to the hospital. We found out that Handmaker had left in the original catheter that she got from the ER at UMC. The doctors at UMC were so shocked at her condition when she arrived to the ER with multiple bruises that she didn't before going to Handmakers. Some of the bruises were on her upper thighs. My aunt told me that she had been inappropriately touched in her private parts. When we took her to UMC, the ER doctors actually submitted a report to the local authorities for elderly abuse. During the time my aunt was at Handmakers, one CNA told me that my aunt didn't like her. My aunt is a sweet elderly lady who is always friendly and kind. Apparently some of the CNA's had handled her roughly. When she got there, she was made to bear weight on her broken legs until the physical therapist determined that she was not to bear weight on her legs several days after she had been admitted. DO NOT take your loved ones there!

    Think twice, read this and research before putting your loved one in Pueblo Springs Rehabilitation center.

    After almost a months stay in the ICU due to complications from Parkinsons Disease and being neglected at an Assisted Living Home, my mother was released to Pueblo Springs for rehab.

    Immediately, i had to have a discussion with the nurses about how Parkinsons has now diminished her upper body strength and how she would need help eating. She is not even able to hold a glas to her mouth without spilling everywhere. That conversation with then went unnoticed and the 2nd day I had to have this conversation once again. On the 3rd day I raised my voice to the nutricianist and that seemed to temporarily work.

    They continually lost her clothes and then put her in a hospital gown.

    I then noticed the rash she had after being taken out of her last home, reappear and by the time I saw it, it had covered 75% of the back of her body. This did not re appear overnight. I immediately contacted the head nurse. She told me she would contact the Dr. right away and get her medical help. That never happened.

    Prior to release day to her new home, the facility and I had discussed the plans of needing the necessary Oxygen at her home before her arrival, having medications in hand and RX's written for all of her medications sent to the caregiver in advance of her arrival.

    On release day, she arrived at her new home without oxygen there, only a portion of her meds in hand and NO RX's for any necessary medication. She is diabetic and was sent home with 2 injections available. Not even 24 hrs. after her release she sits in her new home with elevated Blood sugar and no Insulin left. We are hoping the Dr. Gets to her in time to help us or she will end up back in emeregeny where we started.

    This was totally avoidable, and very neglectful of the Nursing facility. The last words I heard from the Social Services office was that The Dr. did not feel he needed to prescribe any other medications to her other than the 3 I had finally recieved after jumping up and down an demanding. Wouldn't you know, The Dr. did not find it necessary to prescribe Insulin to a Diabetic upon release!

    Think Twice about putting anyone here. Worst medical experience I have ever had to deal with!

    This was my second visit, the first I was out of there in the morning, what the problem is they need to watch there night staff,because its a ghost town on the west side,this was my second time there,I went there because I went to a few others an this place was alot cleaner then the others,in my book they have step it up a little better then the last time I was there.injoyed the techs,an few of the nurse ratchets ..my favorite nurse is nollem

    The Foothills Rehabilitation Center used to be a great home… When it was Posada. The new owner (Avi Rothner) thinks a remodel should be enough to keep this place bringing in new residents. New residents should also know that the new owner is downstaffing and overworking/underpaying folks that love and care about their jobs and the people they take care of. A privately owned nursing home is just that, private, FOR PROFIT. If you have loved ones that need the best care, this used to be the place to go. Now overworked, angry employees will be taking care of residents. I don't know about you but I pay my employees what they are worth, to keep my business running successfully. They hire administrators that fire the old staff to give their friends jobs.

    It's all about numbers here and my father will be removed from here ASAP. It took a little while to realize, but after speaking to the employees and other residents, this is not the place for my father. I truly wish the best to those hard working individuals that actually care about people, all of the workers there are so nice and it's unfortunate that they are ruining the MEDICAL field with visions of a privately owned "business". Euthanasia is a better alternative than a privately owned nursing home.

    This facility is HORRIBLE… The management is so unprofessional. These people do not come out with the truth it is as if they are trying to cover up alolt of their mistakes. My family member never gets walked she is constantly in a wheelchair. The smell is horrible and the patients wheel themselves around like they are lost. You never get a straight answer out of anyone, like I said it's like they are all trying to cover something up. If the patients are'nt in there for loosing their minds, by the time they get out if they ever do they would loose it. Horrible place.

    My husband was in Pueblo Springs for only 2 nights - that was enough. It was under staffed and there was only one aide on the entire floor - the care was minimal When he did not answer his phone I called back and asked them to check to see what was wrong. I was told the phone was by his bed and he could reach it. When I visited the next day, the phone was by his bed but it wasn't plugged in!

    My mother was at ManorCare twice and each time I found the place to be the best. The staff treated her like she was the most important resident there! they answered her call light quickly, never made a mistake in the mmedications and kept her room spotless. The food was great. I had a few meals there myself and thoroughly enjoyed it. Since my wife and I visited daily we were able to see exactly how the place was run and we were inpressed. I highly recommend ManorCare to everyone.

    I wish we had known how short handed the staff is because nurses and aides were not showing up for work, and were quitting. Although there were some exceptional staff members at Pueblo Springs, none of the ones I dealt with were working with patients regularly, they were all in offices and administrative roles. The only nurses on the floor were LPNs and the only RNs I met were administrators. This might be fine, but the only time the nurse was EVER in the room was to pass out medications. Other than that you could usually find them at he nurse's station. Patients were cared for solely by aides, or "CNAs" and there were so few of them that the wait time for care such as a clean "diaper" was often cruel, ranging to as much as more than an hour. My father was so happy to leave this facility that he didn't mind waiting the ten hours in the ER it took him to get a bed in the hospital.

    If there were an option of less than a star I would have elected that option! If you care anything about yourself or loved one this is not the place!!!! My father was sent here and was quickly removed!!!! I was promised this and that and it sounded great. all of those promises were not met in fact my Father was placed in an adult diaper and ignored and was not provided a telephone by his bed as provided making it hard for him to let his family know! He was removed and I called asking to speak to him after he had been gone for an hour and the nurse said he was in the family area eating an orange. 1- he has acid reflux and can't eat citrus and secondly he was sitting by my side whilst I was speaking to them! Care is horrible and when you first walk in the door it reeks of feces!

    Even though many people seem to say bad things about this place, my mother had a three week stay there. The staff was very friendly , esp one of the male LPNs who worked during the day. I suggest people let the staff know up front what you expect. Everything went well and my mother is home again. The only improvement I would suggest is to remodel the entire facility
