    I will give this facility one star, only because there is not an option to give it fewer stars. My 92 year old mother was in this facility in July 2017. She was transferred from the hospital for rehab, after having surgery for a broken hip. While hospitalized, she developed pleural effusions (fluid in the pleural space around her lungs, which caused shortness of breath and difficulty breathing). She had bilateral thoracentesis in the hospital to remove the fluid, and was transferred to LifeCare in good shape, and breathing fine. She arrived at LifeCare on a Friday. The physical therapy staff did not work on the weekends, so other than being pushed in a wheelchair to the restroom and to the dining room for meals, she was not allowed to get out of bed. "Physical therapy" was to be given 4 times a week, for less than an hour each time, but as you will see below, she only received therapy a couple of times in the 10 days she was there. She was in bed most of the time, which ultimately caused her being rushed to the emergency room. By Tuesday, she was getting short of breath and the LifeCare staff put her on oxygen. No testing was done to see WHY she was short of breath. By Thursday, she was so short of breath and fatigued, she could not do physical therapy. I told the nursing staff that she needed a chest x-ray to see if her lungs had filled with fluid again (since she just had a procedure the previous week for this reason). I was told that the doctor would be notified. Friday - I found out that no chest x-ray had been ordered. I, again, told the staff that testing needed to be done, and was told that the doctor would be notified. Saturday - I could not visit, since I was ill, but later found out that the chest x-ray still had not been performed, even though I had already requested it twice. Sunday, I walked into my mom's room, and she was struggling to breath. I ran out of the room and found her nurse standing just outside of her room, in the hallway. I told him that she couldn't breath, and he said, "She's fine. I was just in there." I ran down the hall for the charge nurse and told her that she needed a chest x-ray and lab work NOW! I was concerned that she either had pleural effusions (again) or a pulmonary embolism (just had surgery, had not been up and moving around and could have developed a clot). I asked that she be transferred to the emergency room. I was told that they needed to talk to the doctor before they would call an ambulance. A chest x-ray and labs were performed at LifeCare, and she was rushed to the emergency room via ambulance with a PULMONARY EMBOLISM and SEVERE bilateral pleural effusions! Neither of these life threatening diagnoses were recognized by the LifeCare staff, and even when I brought them to their attention, they did not recognize that this was an emergency situation. Needless to say, the staff at LifeCare is not trained or equipped to handle any medical issues - even when life threatening issues are staring them in the face. DO NOT trust LifeCare to care for your loved one!! I am in the medical field, and would never recommend this facility to anyone. P.S. My mom was at this same facility three years ago, and the staff administered the wrong medication and almost killed her - major medication error. We said at that time that she would never go back to LifeCare, but, unfortunately, her insurance is very limiting when it comes to skilled nursing facilities, and she chose to give LifeCare another chance. Obviously, that wasn't a good choice.

    One of the worst facilities I have ever been in. We placed our mother here for 2 days and after just those days we pulled our mother and took her home. My mother was there for hospice care and they had her set up for therapy and the food they were serving her was not appropriate for her. Do not put your loved one in this facility

    Horrible facility. Upon first glance, this facility looks like a country club, but after my aunt spent a few days here their true colors were revealed. It consistently took up to 1 hour for her to receive her medications for pain. She also hadn't been bathed in a week, and many times was left with her undergarments unchanged and sitting in a wet or soiled adult briefs. When her physical therapy insurance benefit ran out, she was left in her recliner chair for 48 hours. Very sad. Nurses told her she was dramatic, and asked my mother to leave because she acts differently when my mom was there. Read all the reviews BEFORE letting loved ones come here. They are understaffed and their is no communication between departments.

    this facility had bed bugs which they confirmed I reported as accurate by means of their service dog sniffing the bed bugs out of room 110. This facility inappropriately allowed a resident to wheelchair another resident outside to smoke, to cause the wheel chaired resident to stand up and fall scraping his knee and elbow. This facility inappropriately did not honor a Power of Attorney. Beware of the safety of this facility for your loved one. In addition, this facility was filthy. Complaints are filed with the AZ Dept of Health, Medicare, Social Security and Adult Protective Services.

    This is a premiere care facility. Never smells, always clean. The staff is attentive and speaks kindly to residents. The respect a patient's privacy and is good to keep the family involved in the patient's care. Their social worker is kind and understanding. The business office operates efficiently but with care. The nursing staff and caregivers are never speak down to the residents. They have well planned activities and many visits from outside volunteers that help break up the long days for those confined to their beds. They encourage residents to be be active. The residents are all bathed, groomed and dressed in clean clothing. My father was cared for here until he passed. The nursing staff did everything they could be help him and the family to be comfortable during his last days. My husband is now being cared for here and he is getting the best care Scottsdale can offer. Because they are a combination long term care and rehab facility they avoid the pitfalls of many other places. They do their best to keep life interesting for the residents. Kudos to the administration for running a classy facility.

    Scottsdale Village Square has high employee turnover and an alarming number and degree of violations. It has a total of 27 citations as reported by the Arizona Department of Health Services and also has two civil penalties covering the years 2012 and 2013.

    The grounds are older but nice. The level of care and number and type of complaints raise substantial concerns. While all citations apparently require corrective action, SVS did not improve over 2012 and had even more citations in 2013, including on resident care and dignity.

    If I could gice no stars I would. Avoid this place. The nursing care is horrible. Most all of the staff are contingent employees. One night a "new" nurse came in to hang my IV (the reason I was sent there) and within five minutes I couldn't breathe, my tongue was swelling and my body was one huge hive. I called the staff and nobody answered until I literally yelled at a passing in the hall employee (not even on the nursing staff!). He saw the distress I was in and ran out of my room yelling "emergency, emergency, room xxx". By this time (and myself being a RN), I had pulled the IV, but the symptoms persisted. The two nurses who came into my room stupidly asked, "What's wrong, Mrs. X?" Were they serious? They couldn't see the hives/rash on my body, nor that I could barely speak due to the throat swelling? Yup. They refused to call 911 and kept telling me to relax and I'll feel better! Again, were they serious? Yup! When some of the smoke settled, I asked what was in the IV bag they hung because clearly I had a severe allergic reaction. I was told it was the proper medication. I asked to see the IV bag that the nurse ripped off the IV pole and she said that never happened. When I continued to point things out and making it clear that I was NOT hallucinating, I was told that the IV bag was already in the dumpster outside so there was no way to tell what was in there! This is clearly in violation of state laws and nursing practices, Medicare rules, etc., etc., etc. Needless to say, by morning I was out of there.

    I was a resident in this facility for two and one-half days, having been transferred to another skilled nursing facility. The room that was assigned to me was full of bugs and despite complaints to nursing to either spray the room or transfer me to another one, fell upon deaf ears. It took staff forever to answer the call button. Upon my first day there and bing on the phone with my son, he was told that I may have to hang up because I called for assistance to the bathroom. Over two hours later, and after a major accident, someone finally came to my room but were put off because now they had a mess to clean. Also, the toilet backed up and for the entire time I was there, nobody came to do anything about that, either! The first day there they failed to feed me, stating the kitchen was not made aware I was there! The next morning, no breakfast, either! The following day, I had a doctor appointment which they scheduled for 7 a.m., meaning we had to depart the facility at 6:15. I am also a diabetic. They gave me insulin at 6 a.m. telling me breakfast was on the way. It never came and my requests for food of ANY type to take with me also fell upon deaf ears. We ended up being gone for over seven hours, all the time being without food or drink. I was becoming ill from not eating, having insulin, and the extreme heat here in AZ. When my son heard about this he left his job, flew through the streets of AZ to snatch me out of this place. Yeah, the photos on their website may look pretty but the service is completely at the other end of the spectrum. If youtruly love your loved ones, stay away from this place. You -- and they -- will definitely not be happy there.

    I was a patient at this facility, having chosen it based on "positive" on-line reviews and recommendations from many people. They must have been deaf, blind, and lost their taste buds. The photos and videos on their website are very deceptive as the place looks absolutely nothing as their photos depict. The dining room in their photos looks like a "formal" dining room, nice table cloths on every table, "formal" chairs (wood, padded backs and seats), when in reality there are only 14 NONmatching tables and EIGHT matching chairs, less than one at each table! Any windows have CLOSED blinds so it is dark and depressing in there. The food is HORRIBLE. Prepared/processed food products, canned veggies and fruit; toast is ALWAYS burnt. One day they ran out of juice so they mixed together some apple, cranberry, orange and prune to make a concoction in enough quantity to serve all the patients who wanted "something" to drink. The menu states milk with every meal but we never got any. The halls are crowded with equipment. It would take no less than 35 minutes for call lights to be answered. The videos/photos on their website shows patients happily strolling outside on the grounds; do not count on doing anything like that because it just is not going to happen! The rooms are private, if you can honestly call them that (??) or semi-private "jack and jill" rooms meaning there are two patients per room with a "common wall and bathroom" shared by FOUR patients. These patients can (and do) have access to the adjoining rooms! Unfortunately, I had an "obnoxious" woman in the adjoining room that would enter my room through the bathroom and stand at the foot of my bed and just stare at me. She would do this at any time of day and night! Do you know how scary it is to be awakened by having a stranger standing at the foot of your bed? how VIOLATING that is? and to make matters worse, she would then start yelling and screaming at me for having to use HER bathroom! There are no locks on the doors and every time I would be in the bathroom she would walk in on me begin one of her rants, and by the time any staff would answer the call button in the bathroom she would have retreated into her room. Fortunately, there was one time she didn't quite make it out in time and was caught by nursing staff. When confronted and warned that she cannot do this she flat out lied that she was doing anything and wouldn't even dream of doing anything! This place is NOT a rehab center, either, folks. It is a NURSING HOME and has all the earmarks of your stereotypical nursing home. Unless this is the type of place you care for your loved one to be at, stay away from here. You really do not love your loved one if you do place them here as this is nothing less than substandard care and living conditions. You've been warned! Make sure you read ALL reviews and actually physically visit any place before making any commitments for your loved ones!

    My 84 yr old Mother spent 3 days in the hospital before being transferred to Life Care for physical therapy and monitoring before she could have a Vertebroplasty done on a Spinal Compression Fracture to the upper spine. After basically 3 days of "observance" by P.T. and nursing staff, they determined she should be discharged in only one week… Our appeal was immediately denied, and 2nd appeal also immediately denied stating she was "high functioning". This was done before she met with the Spinal Specialist who ordered the Vertebroplasty surgery for the upcoming Friday. They based this on the fact she was WALKING. Her legs weren't broken! She walked because she didn't want to get pneumonia as she did 3 years earlier when placed in the same facility. You would think her spinal specialists should have been consulted, but they were not. I am beyond furious with the callous, premature, ill-investigated, failure to care for my Mother as she deserved. We had to PAY to keep her there until her surgery and follow-up until we could find a suitable care facility. I will never allow my Mother to return to Life Care of Scottsdale as they failed her in my opinion. I was told they had a "D" rating… I understand why. Because she smiled and addressed everyone "sweetheart" they assumed she was doing great. She was in great pain but tried to avoid pain killers due to problems with constipation. She tried hard not to "bother" anyone, and therefore, they ignored the greater picture. I'm still furious!!! Oh, did I mention she fell again - at Life Care - the morning I discharged her????

    My mother is at this rehab facility now. We are in the process of looking for a group home for her. The faster we can get her out of there, the better. I feel like she's in jail. They allow her to lie in a wet bed. They threw her dinner away before she was finished. The food is horrendous, The nurse had her at the station for over an hour in her wheel chair because they said she tried to get out of the bed. (She cannot walk so it would be difficult for her to have tried that!) After speaking to the nurse on staff, we got a different report then what we've seen. It is a nightmare. Do not go there if you can find anything else.

    Check out az.gov elder care and find the right place. Stay with your parent for as long as you can to do your own advocacy. Don't leave her/him alone in this place and make sure you show that you are watching every minute!

    This is not a quality facility as reflected by their ratings. I have known several people who were placed in this facility for a brief time and did not receive good care. Their families moved them to other facilities. Consider other options before placing anyone in this substandard facility.

    After a car accident I was too weak to go home, given the option of two other nursing homes or Scottsdale Heritage, I went here as they are 5 star rated and all private rooms. It's a beautiful facility, the staff there are smart and familiar with care required to get me home back to my family. I was unlucky at first to get to Scottsdale Heritage, but once there, my luck changed and now I am home and happy with my beautiful family in time to enjoy the holidays with. To all the caring nurses and therapists I say thank you…

    Avoid this home like the plague. My grandma has COPD and went to Sante to "build back up her strength" after being released from the hospital. They neglected to let us know that none of the staff knew how to work a bipap machine. After being there a couple of days she was right back in the hospital. She then went back after being released from the hospital and was doing great after being at the hospital; however, after being at Sante for a week with no bipap once again over night she went back down hill due to her Co2 levels being too high, The nurses and the facility did not listen to our wishes of having her go back to the hospital and instead gave her drugs to help with her "shakiness" when all she needed was to get on the hospital's bipap. They allowed her oxygen levels to drop into the 60's overnight and did not call 911 like they said they would. Finally upon thinking that my father was a dr. because they are clueless who the dr.'s are they called 911 under his instructions that she needs to get to the hospital. They are all clueless there and only want your money. They neglected to properly care for my grandma and completely lie to you in the process. I highly recommend that you avoid Sante at all costs!!!!

    This is the best Nursing home my Dad had ever been in for rehab. Recently he had a hip surgery and needed a rehab, These people were fantastic! The staff was always helpful and positive, the Therapy team are like Angels and the Nurses take the time to help with whatever you need. I am so happy we chose Life Care Center of Scottsdale and I would recommend it again to everyone who needs a Nursing Home or rehab.

    Nursing staff sincere, professional, caring and attentive. Doctor attending to my loved one outstanding, personal attention and special care to not overmedicate. CNA staff seems a bit amateur and needs to work on better attention to detail and timeliness of patient personal care. Ambience much nicer and appearance much cleaner with new laminate/vinyl flooring. Non-fabric patient recliners would be more hygenic. Most staff goes above and beyond. Anyone working in this type of job is an angel from God.

    My mother called me from this facility asking me to call the main number to find someone who will come to her room to help her. The call button was not answered. She laid in a soaking wet bed and was so cold her teeth were chattering. Nurses say they will come back to change her, but many times after an hour, we have to summon them again, and then they are grouchy about us calling. I found it easier to change diapers myself. Feces are left on the toilet seat and around the base from splashing. Many of the staff act as if you are bothering them,
