This place is so bad that I cannot even submit enough words to say how bad. We were not greeted upon intake for four hours, and then only after my sister, who is a nurse demanded to be seen. The nurse was the rudest, most arrogant person I have had the displeasure of meeting. I don't even know her name. She didn't tell us. The place reeks of urine upon entering. The beds and bedding should be an embarrassment to the owners of this place. The toilet never stopped running the entire night. My husband spent 18 hours here after being admitted for skilled nursing/rehab after being hospitalized for almost 6 months, in three hospitals, in three Arizona towns. He was not given his medication and had to be transported via ambulance to a 4th hospital for another two weeks because of their neglect. While I was there, there was a patient down the hall that had fallen and no one even knew how long the poor woman had been on the floor. Pure neglect. DO NOT GO HERE!

Immanuel does not return refundable security upon the death of a tenant
My Uncle lived at Immanuel from 1996 until his death April 25, 2014. In closing my uncles affairs we determined that he had made a $300 refundable security deposit. August 26, 2014, we requested the refund. We received several phone calls that the refund had been issued to his wife in 2002. (His wife died in 1998).
We have received no written response from the facility or parent company.) The facility is in violation of Arizona Statutes requiring a written response to a refund request.
Our concern is that when tenants die, the company keeps the security deposit
rather than returning it to the family

I was extremely disappointed with the level of care that my father received at this facility.
He had a stroke approximately 4 years prior and was coming from a facility that did not have skilled nursing. Upon examining The Forum at Desert Harbor's facilities and reviews I believed it would be a good fit for his needs.
In the month that he was there, they kept my father on a puree diet because they said they could not clear him for solid foods (even though I repeatedly asked them to work with him so they could). We also had to fight for them to get him up from bed for eating and to keep him hydrated.
I asked for them to repeatedly work with him in occupational and physical therapy but they said they could "only do so much" and his state declined while he was there.
Finally, my Mother brought to their attention that he did not appear to be doing well and that "something's wrong" and it took another day before they called 911 and had him transported to Boswell with a urinary tract infection, a fever, and a blood clot in his arm. Boswell staff reported that they believed he was not being taken care of properly and that he had sores from not being cleaned.
We have since moved him to Plaza del Rio. In the week that he's been there he is sitting up straight, able to communicate, and has been cleared to eat solid foods. HE IS IN A MUCH BETTER PLACE NOW.

This place treated my mom very, very well. She spent a month there for rehab and physical therapy. The entire staff was pleasant, efficient and courteous. The facility was comfy and the food was great. My mom and i enjoyed movie and popcorn day . If you have to be stuck
in rehab, this is a great plce to be.

I have had 3 experiences with the loving staff at Peoria Good Shepherd-Good Samaritan Society. One was for my uncle who was in there short term after a stroke, the other my Grandma who was in there almost 2 years. Later my Uncle also used the hospice side during his last 3 weeks of life. I have never had a problem with any staff or admin. There Activity team was amazing at trying to get the majority involved with Ice cream socials (w/entertainment), outings, Holiday parties, Karaoke, Sittercise, card games, bingo. Plus, the facility has a chapel, for everyone. There is an all denomination Sunday Service with Pastor Tom. A Loving Pastor who always sends a great message and also conducts a Bible study for everyone who wants to join in during the week. My brother was at Immanuel Campus of Care in 2007. It was the worst nursing home I have ever been in. When he passed away, he was so full of bed sores he couldn't even move. They gave him medications we requested not to be given to him because it made him hullucinate. When we reported one of the aids was abusing him, they began giving him medication so he slept all the time. He never had water to drink. His diaper was always soiled, and they never came into change him unless we requested it. He couldn't eat on his own, but no one came in to feed him. Sheets on bed were only changed once a week. Stool on the bathroom floor that was left there for several hours. He fell several times and had to wait at least half hour for someone to help him up. No blankets on cold nights, only a sheet. Took almost an hour to give him pain medication after we requested it. I think the conditions at this nursing home was horrible. The patients that didn't have visitors were really neglected. The staff just didn't seem to care about the patients at all.

My brother was in this facility a few years back, and I think it is a horrible place. I would see patients in wheelchairs crying because they needed to go to the bathroom, and their cries were ignored. My brother developed bed sores that covered a large area of his back. When he couldn't feed himself, I would go ask the nurse if he had eaten, and she said they gave him a tray. They just dropped it off, but no one ever feed him, and he couldn't do it himself. The tray was still full when they picked it back up. He would lay in soiled diapers until I would come up in the evening and demand they would change him. I wouldn't take a dog to this nursing home.

worst place!!! nurses can't answer any of your questions regarding your parent, they tell you to talk to the social worker. social worker doesn't return call nor does the doctors return your calls. when you go visit at night and talk to the night nurses they tell you they don't know you have to talk to day nurses.

I was there for about 11 days following knee replacement surgery. I could not have gone to a better place. I won't name the people (on the short stay wing) but without exception, they were all WONDERFUL. I never had to wait more than 30 seconds when I rang the bell for something. They were always smiling, personable, joking, caring, never complained of my requests, etc. Melissa and Aubrey in physical therapy whipped me back into shape and even thought it was very difficult, I fell completely in love with the 2 of them and the staff on my wing. PS: I was in room 114 and based on my care I got, I would recommend this facility to anybody.

i use to volinteer at sungrove it was so much fun i loved it and the elders were so nice to me i use to walk the around in the wheel chairs and play activitys with them.

If you are interested in finding a job don't look here the management is awful and the staffing coordinator lies and put you on the schedule with out asking. Also if you do work or go to work here always copy your schedule as they make changes with out notifing you. As far a patient care goes the on the floor staff do their best with what is available but as far as managment goes they only care about making money they don't care about the residents. And also they say this is a christian facility guess again they treat the staff like crap and the residents Management sucks!! I am not making this stuff up i would hate to see anyone else go through the stuff I have.

If I could give 0 stars, i would. Worst place to send a parent. If you want them to be neglected and see nurses without sensitivity training then this is your place. When the patient to nurse ratio is 40:1, stay away!
