my wife was at this facility after knee replacement surgery for physical therepy. when she had to call me to call cogburn and ask the nurse to answer the call button it upset me. she had been calling the nurse for 4 hours for a bedpan and pain… Full Review

This facility appears to be clean, but thats about the only good thing about it. IT IS DANGEROUS. DO NOT SEND A FAMILY MEMBER TO THIS FACILITY. My family member was neglected physically and medically by staff and doctors.… Full Review

Family member is a resident who is on hospice. The facility is nice and clean with homelike appearance. I feel my loved one is usually well cared for. My complaint is that nurses from the monthly meetings do not communicate to the staff… Full Review
